Akut biliyer pankreatitte laparoskopik kolesistektomi

Özet

Amaç: Akut pankreatitin en sık nedeni safra taşı hastalığıdır. Pankreatitin tekrarını önlemek için kolesistektomi yapılması gereklidir. Bu çalışmada amacımız safra taşına bağlı pankreatitli olgularda laparoskopik kolesistektominin sonuçlarını değerlendirmek ve semptomatik safra kesesi taşı nedeniyle laparoskopik kolesistektomi uygulanan olguların sonuçları ile karşılaştırmaktır. Hastalar ve Yöntem: Laparoskopik kolesistektomi uygulanan akut biliyer pankreatit tanılı 86 olgu (grup I) ile semptomatik safra kesesi taşı nedeniyle laparoskopik kolesistektomi uygulanan 93 olgu (grup II) çalışmaya dâhil edildi. Hafif pankreatitli olgular ilk yatış süresi içerisinde operasyona alındı. Akut biliyer pankreatitli olgular yaş, cinsiyet, üst karın operasyonu öyküsü, pankreatitin şiddeti, lökositoz, ameliyat süreleri, açık ameliyata geçme oranları, dren kullanımı, ameliyat sonrası komplikasyonlar, mortalite ve hastanede postoperatif yatış süreleri açısından semptomatik safra kesesi taşlı olgularla karşılaştırıldı. Bulgular: Gruplar arasında açık operasyona geçme (p=0,3418) ve yara yeri enfeksiyonu (p=0,3230) açısından anlamlı fark yoktu. Hastanede postoperatif kalış süresi açısından gruplar arasında fark yoktu (p=0,2406) (grup I'de medyan 3 gün, grup II'de medyan 2 gün). Akut biliyer pankreatitli hastaların 14'ünde (%16,2) şiddetli pankreatit vardı. Bunların 7'sinde (%50) açık operasyona geçildi. Hafif akut biliyer pankreatitli hastaların ise 3'ünde (%4,1) açık operasyona geçildi. Şiddetli akut biliyer pankreatitli hastalarda açık operasyona geçme oranı anlamlı olarak yüksek bulundu (p<0,0001). Hafif akut biliyer pankreatitli hastaların 68'i (%94,4), şiddetli akut biliyer pankreatitli hastaların ise 12'si (%85,7) ilk yatışta operasyona alındı. Sonuç: Hafif akut biliyer pankreatitli hastalarda pankreatit tablosu geriledikten sonra laparoskopik kolesistektomi güvenle uygulanabilir. Ancak akut biliyer pankreatitli hastalarda laparoskopik kolesistektominin uygulanabilirliğini etkileyen en önemli faktör hastalığın şiddetidir. Bu nedenle şiddetli akut biliyer pankreatitte laparoskopik kolesistektominin, pankreas ve çevre dokulardaki enflamasyonun geçmesi için ertelenmesi uygun yaklaşımdır.

Abstract

Purpose: Gallstones represent one of the main causes of acute pancreatitis. The aim of this study was to evaluate laparoscopic cholecystectomy results due to acute pancteatitis and compare the surgical results with patients who are operated on symptomatic cholecystolithiasis. Patients and Methods: Eighty-six patients have underwent cholecystectomy because of acute biliary pancreatitis (Group 1), 93 patients have underwent cholecystectomy because of symptomatic cholecystolithiasis (Group 2). Patients with mild acute pancreatitis were operated in the initial hospitalization. Groups were compared by age, race, previous abdominal surgery, severity of acute pancreatitis, leukocytosis, operative time, conversion to open surgery, postoperative complications, drain management, mortality rates and hospitalization period. Results: There were no differences regarding conversion rates to open surgery (p=0,34) and wound infection rates (p=0,32). There were no differences regarding postoperative hospitalization period in both groups (p=0,2406) (median 3 days in group 1 and 2 days in group 2). Fourteen patients had severe acute pancreatitis (16%) and open surgery was performed in 7 (50%) of these patients. In mild acute pancreatitis patients only 4 (4,1%) conversion to open surgery had occured. Cholecystectomy was performed ,in 68 (94,4%) mild acute pancreatitis patients and in 12 (85,7%) severe form in the initial hospitalization. Conclusion: Laparoscopic cholecystectomy can be performed confidently during the index hospitalization after amelioration of the pancreatitis. However, the most important factor that affects the feasibility of laparoscopic cholecystectomy is the severity of the disease. Therefore, laparoscopic cholecystectomy should be postponed until the resolution of the pancreatic and peripancreatic inflammation.
