Protrombin 20210 G/A ve MTHFR C677T mutasyonları nedeniyle süperior mezenterik ven trombozu: olgu sunumu

Özet

Amaç: Kalıtsal trombofilili bir hastada gelişen süperior mezenterik ven trombozunun tanı ve tedavisini tartışmak, bu hastalarda koruyucu tedavinin önemini vurgulamak. Gereç ve Yöntem: Protrombin 20210 G/A ve MTHFR C677T mutasyonları olan ve süperior mezenterik ven trombozu nedeniyle acilen ameliyat edilen bir hastanın peroperatuvar süreçleri incelendi. Bulgular: Süperior mezenterik ven trombozu tanısı konulan hastaya laparoskopik yöntemle parsiyel ince bağırsak rezeksiyonu ve primer anastomoz yapıldı. Postoperatif oral antikoagülan tedavi başlandı. Hasta sorunsuz taburcu edildi. Sonuç: Ülkemizde, ender de olsa, kalıtsal trombofili nedenleri arasında protrombin 20210 G/A ve MTHFR C677T mutasyonlarına rastlanmaktadır. Tekrarlayan venöz tromboz atakları geçiren olgularda protrombin 20210 G/A ve MTHFRC 677T mutasyonu düşünülmelidir. Bu olgularda oral antikoagülan tedavinin ömür boyu uygulanması önemlidir.

Abstract

Aim: To discuss the diagnosis and treatment of superior vein thrombosis in a patient who has hereditary thrombophilia, to point out the importance of preventive treatment in these patients. Materials and Methods: We investigated the whole peroperative period in a patient who underwent emergency surgical treatment for superior mesenteric vein thrombosis due to prothrombin 20210 G/A and MTHFR C677T mutations. Results: We performed partial small bowel resection and primary anastomosis laparoscopically with the diagnosis of superior mesenteric vein thrombosis. Oral anticoagulation treatment was initiated at the postoperative period. The patient was discharged without any problem. Conclusion: Although it is quite rare, prothrombin 20210 G/A and MTHFR C677T mutations may be found in Turkish population. In patients who have multiple thrombotic events, prothrombin 20210 G/A and MTHFR C677T mutations should be considered. Life-time oral anticoagulation therapy is important in these patients.
