Perioperatif ibuprofen tedavisinin cerrahi stres üzerindeki hormonal ve metabolik etkileri

Özet
Amaç: Cerrahi stres, hasarın boyutu ile orantılı olarak hormonal ve sitokin yanıtlarına sebep olur. Siklooksijenaz inhibitörlerinin perioperatif alımını, nitrojen kayıplarını ve sitokin üretimini azaltır. Bu çalışmanın amacı, siklooksijenaz inhibitörü olan ibuprofen'in cerrahi stres üzerindeki hormonal ve metabolik etkilerini klinik olarak değerlendirmektir. Gereç ve Yöntem: Tiroidektomi geçiren 20 hastada prospektifklinik çalışma gerçekleştirildi. Hastalar randomize olarak ibuprofen fn= W) ve kontrol (n =10) gruplarına ayrıldı. Kontrol grubuna ilaç verilmeksizin, ibuprofen grubu hastalara ameliyattan 12 ve 2 saat önce ve postoperatif3. güne kadar her 8 saatte bir 400 mg ibuprofen oral olarak verildi. Her iki grup hastalardan, cerrahiden 24 ve 2 saat önce, cilt insizyonundan sonra 1. 4. 6.24. 48. ve 72. saatlerde glikoz, CRP (C Reaktif Protein), lökosit, ACTH (Adrenokortikotropik hormon), kortizol için kan örneklen alındı. Bulgular: Preoperatif değerlerle karşılaştırıldığında, cerrahi her iki grupta da tüm endokrin ve metabolik parametrelerde anlamlı değişikliklere neden oldu. ACTH L saatte ve kortizol 6. saatte en yüksek seviyelere çıktı. Birinci saatten sonra plazma ACTH seviyeleri düşerken, kortizol seviyeleri 72 saat kadar yüksek kaldı. İbuprofen grubunda ACTH ve kortizol yanıtları, kontrol grubuna göre daha düşük bulundu (p < 0.001 ve p < 0.001). İbuprofen grubunda glukoz (p < 0.001) ve lökosit sayısı (p<0.001) hafifçe yükseldi. Serum CRP seviyeleri sadece postoperatif l. günden itibaren yükseldi ve 3. güne kadar yüksek seyretti (p < 0.01). İbuprofen grubunda ateş daha az vükseldi(p<0.01). Sonuç: Sonuçta, perioperatif dönemde ibuprofen ile tedavi humoral ve nöroendokrin yanıtları azaltabilir, ancak tamamen baskılayamaz. Çalışmamızda elde ettiğimiz sonuçlar, ibuprofen'in ağır cerrahi hastalarda stres cevabı azaltmada yararlı olabileceğini göstermektedir.
Abstract
Background: Surgical stress induces hormonal and cytokine responses proportional to the extent of the injury. Perioperative administration ofcyclo-oxygenase inhibitors reduces cytokine production and nitrogen losses. The objective of this study is to evaluate clinically the metabolic and hormonal effects of ibuprofen which is cyclooxigenase inhibitor on surgical stress. Methods: A prospective clinic study was performed in 20 patients who undergone thyroidectomy. Patients were randomly divided two groups as ibuprofen group (n= 10) and control group (n= W). In the ibuprofen group, pills containing ibuprofen (400 mg) were administered orally 12 and 2 hours before surgery, and every 8 hours until the third postoperative day. In the both groups, blood samples were collected 24 and 2 hours before surgery and 1, 4, 6, 24, 48, and 72 hours after skin incision for glucose, CRP, leukocytes, ACTH, cortisol determinations. Results: When preoperative values were compared with postoperative values, surgical stress caused significantly changes on the all parameters in the both groups. The highest levels of ACTH and cortisol were at 1" and 6"1 hours respectively. In both groups, plasma cortisol levels remained elevated for 3 days, whereas plasma ACTH levels returned to the basal level at 1 day. In the ibuprofen group, the levels of ACTH and cortisol were significantly less than those of control group (p < 0.00 ? and p<0.001). In the ibuprofen group glucose ievel (p<0.001 ) and count leukocyte (p<0.001 ) increased mildly.The level of CRP increased gradually after first day and were high until 3"' days (p<0.01). In the ibuprofen group the fever increased smaller (p<0.01). Conclusions: In conclusion, in perioperative periodpretreatment with ibuprofen is able to reduce the neuroendocrine and humoral responses, but it can not completely suppress. Our present study suggests that ibuprofen may be useful in decreasing the surgical stress response in the serious patients.
