Özet
GİRİŞ Bu çalışmanın amacı, izole travmatik hemoıoraks gelişen olgularda sistematik yaklaşımı değerlendirmekteydi. GEREÇ VE YÖNTEM Bin dokuz yüz doksan beş - 2003 tarihleri arasında küm ve penetran travma sonucu izole hemoioraks gelişen 103 olgunun tedavi modaliteleri retrospektif olarak analiz edildi. BULGULAR Yüz üç izole hemotorakslı olgunun seksen sekizi erkek (%85,4), on beşi bayan (%14,6), yaş ortalamaları ise 39,4 yıl (17-79) idi. Hemotoraksın, kırk bir (%39,8) hastada sol, altmış (%58,2) hastada sağ ve iki (%2) hastada bilateral olduğu görüldü. Hemotoraks, elli üç (%51,45) hastada kesici delici alet yaralanması (KDAY), kırk dokuz hastada (%47,57) kunt toraks travması ve birinde de (%0,98) yatrojenik yaralanma sonucu oluştu. Doksan dokuz olguya konservatif yaklaşımla torakostomi tüpü uygulandı. Bir olgu acil torakotomiye alındı. Üç olguya ise hematom nedeniyle elektif koşullarda torakototni yapıldı. Yüz üç olgunun 21'ine torakotomi, altısına ise torakoskopi uygulandı. Seksen iki olguya uygulanan konservatif yaklaşım yeterli sonuç verdi. Bir olgu (%0,9) multiorgan yetmezliği nedeniyle eks oldu. Ortalama hastanede kalış süresinin 8,7 gün olduğu saptandı. Operasyona alman olgularda hemotoraks nedeni olarak sistemik arterler (interkostal ve internal mamariyen arter) veya major pulmoner hasar olarak saptandı. SONUÇ Travmatik hemotorakslı olgularda tüp torakostomi sonrası, hasta stabilizasyonuna bağlı olarak, eksplorasyon veya konservatif tedavi karan verilmelidir. Hematomun drene edilemediği olgularda torakoskopik olarak hematom evakuasyonu uygulanabilir.
Abstract
BACKGROUND The aim of this study was to evaluate the systematic approach to the isolated traumatic hemothorax. MATERIAL AND METHODS The treatment modalities of 103 cases with isolated hemothorax was analyzed retrospectively between 1995 and 2003. RESULTS We observed 103 isolated hemothorax cases. Eighty eight of them were male and 15 were female. The mean age was 39.4 years. The hemothorax was left sided at 41 cases, right sided at 60 cases and bilateral at 2 cases. Fifty three (51,4%) penetrating, forty nine (47,5%) blunt, and one (0,9%) iatro-genic traumas with resultant hemothoraxes were investigated. We performed tube thoracostomy in ninety nine cases. One case required an urgent thoracolomy. We operated twenty one cases and thoracoscopy was performed lor six of them. The conservative approach was sufficient for eighty two patients. Mortality was seen in only one case (0,9%), because of a multiorgan failure. The mean rate of the hospital stay was 8,7 days. During operation, we explored for major pulmonary and systemic artery injury. CONCLUSIONS The decision of exploration or conservative treatment must be made according to the patient stabilization status after the thoracostomy tube insertion. Thoracoscopic evacuation of blood clots from hemothoraxes must be attempted during the early posttraumatic stages, especially in cases refractory to classical drainage methods.
