Topikal rifampisin fıtık onarımında cerrahi alan enfeksiyon riskini azaltır mı?

Özet
Amaç: Kasık fıtığı operasyonları sık uygulanan genel cerrahi operasyonlarındandır. Fıtık onarımının tarihsel gelişi- minde birçok cerrahi yöntem denenmiş olup günümüzde en yaygın olarak yama yardımlı gerilimsiz fıtık onarımı yapılmaktadır. Ancak kullanılan sentetik materyallerin enfeksiyona yol açabileceği düşüncesiyle temiz yara sınıfında olmasına rağmen antibiyotik gerekliliği halen tartışma konusudur. Çalışmamızda yama destekli gerilimsiz fıtık ta- miri yapılan hastalarda, yama üzerine lokal yoldan rifampisin uygulanmasının olası enfeksiyona etkisini saptamayı amaçladık. Gereç ve Yöntemler: Hastanemiz genel cerrahi kliniğinde kasık fıtığı nedeniyle opere edilen hastaların dosyaları retrospektif olarak tarandı. Tedavide sentetik materyal kullanılan hastaların bilgileri ve ameliyat raporları kayıt edildi. Hastalar, antibiyotik kullanılmayan hastalar (G1) ve yama üzerine lokal yoldan rifampisin kullanılan hastalar (G2) olmak üzere iki gruba ayrıldı. Postoperatif erken dönemde gruplar arası enfeksiyon oranları karşılaştırıldı. Bulgular: Çalışmamıza dahil edilen 278 hastanın yaş ortalaması 49,6±15,39 olup kadın/erkek oranı 10/268 idi. Dört hastada nüks saptanırken, 22 hastada erken dönemde yüzeyel enfeksiyon, 1 hastada da testis torsiyonu gelişti ve orşiektomi yapıldı. Grup 1 ve Grup 2 arasında cinsiyet dağılımı ve yaş ortalamaları incelendiğinde gruplar arasında anlamlı fark saptanmadı. Fıtık tipleri ve vücut kitle indeksleri de gruplar arasında homojen dağılımlı idi. Postoperatif erken dönemde Grup 1 ve Grup 2’deki enfeksiyon oranları sırasıyla 16/144 (%11,1) ve 6/134 (4,48) olarak saptandı ve istatistiksel açıdan anlamlı fark tespit edildi (p=0,041). Sonuç: Çalışmamızdan çıkan bulgular ışığında yama kullanılarak fıtık onarımı yapılan ameliyatlarda lokal yoldan rifampisin profilaksisinin cerrahi alan enfeksiyonunu azalttığını düşünmekteyiz.
Abstract
Objective: Inguinal hernia operations are common procedures in general surgery. There have been many approaches in the historical development of hernia repair, and tension free repair with mesh is the most common technique today. Although it is a clean wound, antibiotic use is still controversial because of the role of the patch in development of infection. We aimed to determine the probable role of topical rifampicin in patients with tension-free hernia repair and patch support. Material and Methods: The files of patients with tension-free hernia repair were screened retrospectively. Informa- tion and operative notes of patients with synthetic materials were enrolled. The patients were divided into two gro- ups, patients with placebo (G1) and patients with patches with topical rifampicin (G2). Infection rates were compared between the groups in the early postoperative period. Results: The mean age of the 278 patients who were included study was 49.6±15.39 and the female/male ratio was 10/268. There were recurrences in four patients and superficial infection in 22 patients in the early period. One patient had testis torsion and an orchiechtomy was done. There were no significant differences between the groups in terms of age and gender. The types of hernia and body mass indexes were homogenous in the two groups. In the early postoperative period the infection rates were 16/144 (11.1%) and 6/134 (4.48%) in the groups, respectively, with statistically significant differences (p=0.041). Conclusion: We suggest that applying rifampicin locally can decrease surgical field infection in operations where patches were used.
