Özet

Meme kanserli hastalarda lenf nodüllerinin durumu, tedavi ve prognozun belirlenmesinde çok önemlidir. Sen- tinel lenf nodülü primer tümör bölgesinin ilk drene olduğu ve metastazının görülebileceği ilk lenf nodülüdür. Meme kanserinde sentinel lenf nodülü saptanması ve biyopsisi halen klinikte uygulanmaktadır. Planar lenfosin- tigrafi, mavi boya ve intraoperatif gama prob sentinel lenf nodülü ve lenfatik drenajın saptanmasında kullanılan yöntemlerdir. Planar lenfosintigrafi genellikle tüm hastalarda sentinel lenf nodülünü saptar. Fakat sentinel lenf nodülünün gerçek anatomik lokalizasyonu için yetersiz kalabilir veya bazen lenfatik drenaj hepsinde gösterile- meyebilir. Son zamanlarda birleştirilmiş tek foton emisyon bilgisayarlı tomografi-bilgisayarlı tomografi (SPECT/ BT) sistemleri geliştirilmiştir. Bu sistem planar lenfosintigrafide karşılaşılan zorlukların üstesinden gelmeye yardım edebilir. SPECT görüntüleri, BT’den elde edilen anatomik detaylar ile birleştirilince, sentinel lenf nodü- lünün gerçek anatomik lokalizasyonu ve lenfatik harita elde edilebilir. SPECT/BT planar lenfosintigrafinin duyar- lılığını artırır, planar lenfosintigrafi ile kıyaslandığında daha çok lenf nodülü saptar ve sentinel lenf nodülünün gerçek lokalizasyonu cerrahinin planlanmasına kılavuzluk eder. Bu derlemede meme kanserli hastalarda sen- tinel lenf nodülünün saptanmasında birleştirilmiş SPECT/BT’nin klinik kullanımı, planar görüntülerle karşılaştır- ma, bu konu hakkında literatürler ve tecrübemiz sunulmuştur.

Abstract

The lymph node status is very important for the prognosis and treat- ment in patients with breast cancer. The sentinel lymph node is the first node to which lymphatic drainage and metastasis from the primary tumor occurs. The sentinel lymph node detection and biopsy have already been implemented into clinical practice in breast cancer. Planar lymphoscintigraphy, blue dye and intraoperat- ive gamma probe are used for detection of the the sentinel lymph node and lymphatic drainage. Planar lymphoscintigraphy usually determines the sentinel lymph node in all patients. However, planar lymphoscintigraphy is not adequate for the precise anatomic local- ization of the sentinel lymph node, or sometimes lymphatic drainage may not be shown at all. Recently, hybrid single photon emission computed tomography gamma camera-computed tomography sys- tem (SPECT/CT) has been improved. This system could help over- come these difficulties. When SPECT images are fused with the anatomical details provided by CT, accurate anatomic localization of the sentinel lymph node and lymphatic mapping can be obtained. SPECT/CT increases the sensitivity of planar lymphoscintigraphy, detects more nodes compared to planar lymphoscintigraphy, and the accurate localization of the sentinel lymph nodes acts as a guide for surgical planning. In this review, the clinical use of hybrid SPECT/ CT in the identification of the sentinel lymph node in patients with breast cancer, in comparison with planar images, literature about this subject together with our experience were presented.
