Plevral efüzyonların ayırıcı tanısında kompleman C3 ve C4’ün önemi

Özet
Amaç: Plevral sıvı C3-C4 düzeylerinin ve plevral sıvı/serum C3-C4 oranlarının, transüda-eksüda ayrımındaki önemini belirlemeyi ve tüberküloz, malign ve parapnömonik efüzyonlardaki C3-C4 düzeylerini karşılaştırıp tanısal değerini bulmayı amaçladık. Gereç ve yöntem: Yirmi beşi transüda, 45?i eksüda olan 70 hastada serum ve plevral sıvılarda total protein, LDH, kompleman C3 ve C4 ölçüldü. Bulgular: Transüda-eksüda ayrımında plevral sıvı C3 için cutoff değeri 50 mg/dL alındığında, sensitivite %80, spesifisite %100 bulundu. Plevral sıvı C4 için cutoff değeri 10 mg/dL alındığında sensitivite %82.2, spesifisite %88; plevral sıvı/serum C3 cutoff değeri 0.30 alındığında sensitivite %77.8, spesifisite %96, plevral sıvı/serum C4 cutoff değeri 0.20 alındığında sensitivite %88.9, spesifisite %72 bulundu. Eksüda içinde yer alan gruplar karşılaştırıldığında, plevral sıvı C3-C4 ve plevral sıvı/serum C3-C4 oranı tüberküloz plörezili grupta daha yüksekti. Sonuç: Bu bulgular transüda-eksüda ayrımında plevral sıvı C3-C4 ve plevral sıvı/serum C3-C4 oranlarının ümit verici bir yaklaşım olduğunu ve tüberküloz plörezide tanısal değer taşıdığını göstermiştir.
Abstract
Aim: We aimed to express the importance of pleural fluid C3-C4 levels and pleural fluid/serum C3-C4 ratio in transudative-exudative differentiation and to compare the C3-C4 levels in tuberculosis, malignancy and pneumonia effusions, thus detecting the diagnostic value. Material and methods: Of 70 patients studied, 25 had transudative and 45 had exudative effusions. Total protein, LDH and complement C3-C4 levels were measured for all. Results: In transudate-exudate distinction, the cut-off, sensitivity and specificity values are given below respectively for pleural fluid C3-C4 and for pleural fluid/serum C3-C4 levels. Pleural fluid C3: 50 mg/dL, 80%, 100%. Pleural fluid C4: 10 mg/dL, 82.2%, 88%. Pleural fluid/serum C3: 0.30, 77.8%, 96%. Pleural fluid/serum C4: 0.20, 88.9%, 72%. When the exudative subgroups were compared, both the pleural fluid C3-C4 levels and pleural fluid/serum C3 ratios were higher in tuberculosis pleurisy group. Conclusion: The findings suggest that the C3-C4 pleural values and the pleural fluid/serum C3-C4 ratios are encouraging approaches and they have a diagnostic value in tuberculous pleurisy.
