Özet

maç: Tüberküloz plörezi, ülkemizde eksüdatif efüzyonun sık görülen bir nedenidir. Biz de özelliklerini ve tanı yöntemlerini değerlendirmeyi amaçladık. Gereç ve yöntem: Çalışmamıza, tüberküloz plörezi tanısı konulan toplam 154 olgu (20 kadın, 134 erkek; yaş ortalaması 30.7 ± 15 yıl) alındı. Plevral efüzyon daha sıklıkla sağ tarafta (%55.8) saptandı. Olguların %7.8?inde parankim lezyonu plevral efüzyona eşlik etmekteydi. Olguların %96.8?inde (149/154) lenfosit hâkimiyeti vardı ve hepsinde eksuda karakterindeydi. Toplam 89 olgunun plevra sıvı ADA düzeyi değerlendirildi. Bulgular: Seksen dokuz olgunun 86?sında ADA değeri 40?ın üzerindeydi (%96.6). Üç olguda (%3.4) ADA düzeyi 40 IU/L?nin altındaydı. ?ki olgunun balgamında Löwenstein-Jensen besiyerinde M. tuberculosis üredi. Dört olguda, sıvıda kültür pozitifliği saptandı. Hiçbir olguda direkt bakı ile aside dirençli basil saptanmadı. Plevra biyopsisi yapılan 114 olgunun 100?ünde (%87.7) tüberküloz tanısı histopatolojik olarak kanıtlandı. Plevra biyopsisi 55 (%36) hastada ve ADA 41 (%27) hastada tek tanı yöntemi idi. Torasenteze bağlı olarak %9.1, plevra biyopsisine bağlı olarak %3.2 hastada pnömotoraks komplikasyonu gelişti. Plevra biyopsisinin tanısal olması ADA düzeyi (p<0.001) ve sıvı miktarı ile ilişkili idi (p=0.04). Sonuç: Plevra biyopsisi, tüberküloz plörezi tanısında başarılı ve komplikasyon oranı az bir yöntem olarak kullanılmakla birlikte, sıvıda lenfosit hâkimiyeti saptanması ve ADA aktivitesi yüksekliğinden tanıya yardımcı yöntemler olarak yararlanılabilir.

Abstract

Aim: Tuberculous pleurisy is a common cause of exudative effusions in our country. Our aim was to determine the features of tuberculosis pleural effusion and evaluate the diagnostic methods. Material and methods: 154 (20 female and 134 male; mean age 30.7 ± 15 years) cases diagnosed as tuberculous pleurisy were included in our study. Pleural effusion was determined predominantly in the right side (55.8%). Parenchymal infiltration accompanied to the pleural effusion in 7.8% of the cases. Lymphocyte predominance was seen in 96.8% (149/154) of the cases and characterized as an exudate in all of them. Pleural fluid adenosine deaminase (ADA) levels were evaluated in 89 cases. Results: ADA level was over 40 IU/L in 86 (96.6%) cases and under 40 IU/L in 3 (3.4%) cases. M. tuberculosis was isolated in Lowenstein culture of the sputum in 2 cases. Pleural fluid culture was positive in 4 cases. ARB positivity with direct examination wasn?t observed in any of the cases. Histopathological analysis confirmed tuberculosis diagnosis in 100 (87.7%) among 114 patients that have undergone pleural biopsy. Pleural biopsy and ADA level were the only diagnostic methods in 55 (36%) and 41 (27%) cases, respectively. Pneumothorax was seen as a complication due to thoracentesis in 9.1%, and due to pleural biopsy in 3.2% of patients. Diagnostic success of pleural biopsy was related to ADA level (p<0.001) and the amount of pleural fluid (p=0.04). Conclusion: Although pleural biopsy has been used as a frequent and successful method with low complication rate, lymphocyte predominance and high ADA activity can be used as supportive methods in the tuberculous pleurisy diagnosis.
