Eosinophilic Pleural Effusion: Pulmonary Hydatid Cyst Ruptured Into the Pleural Space

Özet

Kırkbeş yaşında erkek olgu, ateş, öksürük ve nefes darlığı ile baş- vurdu. Solunum sesleri sağda azalmıştı. Akciğer radyogramında sağda 4. ön kot kavsine kadar uzanan homojen dansite artımı izleniyordu. Torasentez ile alınan sıvıda eozinofil hücre hakimiyeti görüldü. Bilgisayarlı toraks tomografisi kesitlerinde sağ plevral aralıkta masif plevral efüzyon, sağ alt lobda atelektazi ve sıvı içersinde anteriyorda yerleşimli membran benzeri oluşum dikkati çekti. Batın ultrasonografide sağ hemitoraksta sıvı ile birlikte karaciğerde sağ lob yerleşiminde 50 mm çapta kistik lezyon raporlandı. Kist hidatik ELISA 164.49 NTU (pozitif) gelen olguda operasyonda perfore olmuş kist hidatik saptandı. Eozinofilik efüzyonların nadir nedenlerinden olması nedeniyle sunuldu.

Abstract

A 45-year-old man was admitted with fever, cough and shortness of breath. Physical examination revealed decreased pulmonary sounds on the right hemithorax. There was an increased homogenous opacity till the 4. anterior costa on the right side at chest x-ray. Eosinophilic infiltration was observed in the pleural fluid. Computed tomography of thorax revealed right sided masive pleural effusion, right lower lobe athelectasis and a membranous appearence inside the pleural fluid located anteriorly. Plural fluid in the right hemithorax together with a 50 mm cystic lesion in the right lobe of liver was reported at abdominal ultrasonography. The hydatic cyst ELISA test was positive (164.49 NTU). A perforated hydatic cyst was determined during surgery. This case is presented as a rare case of eosinophilic pleural effusion.
