Özet
Endobronşiyal intratumoral kemoterapi ( EİTK), intravenöz yoldan kullanılmakta olan bir veya birden fazla sitotoksik ilacın fleksibl bir bronkoskoptan kateterli bir iğne aracılığı ile doğrudan doğruya tümör dokusu içine enjekte edilmesinden ibaret yeni bir girişimsel bronkoskopi yöntemidir, intratumoral kemoterapi diğer endoskopik ablasyon yöntemlerinde olduğu gibi tümör kitlesini basitçe ortadan kaldırmayı amaçlayan bir yöntem olarak düşünülmemelidir. EİTK bir taraftan tıkanmaya yol açan endobronşiyal tümör kitlesinin ortadan kaldırılmasını sağlarken; diğer taraftan da brakiterapi ve fotodinamik tedavide olduğu gibi normal hücrelere zarar vermeksizin kanser hücreleri üzerinde spesifik sitotoksik bir etki yapar. EİTK, intravenöz kemoterapiden sadece veriliş yolu bakımından fark gösteren gerçek bir lokal kemoterapidir. intratumoral enjeksiyon ile terapötik maddelerin lokal olarak verilmesinin üstünlüğü tümör dokusu içinde ilacın eşit dağılımı ile beraber intravenöz yol ile asla elde edilemeyecek yükseklikte bir konsantrasyonun sağlanması; buna karşılık önemli sistemikyan etkilerin ortaya çıkmamasıdır.. Bu derlemenin amacı yöntemin tekniğinin ayrıntılı olarak tanıtılması ve girişimsel bronkoskopi yöntemleri arasındaki yerinin belirlenmesidir.
Abstract
Intratumoral injection of one or several conventional cytotoxic drugs directly into the tumoral tissue through a flexible bronchoscope by means of an ordinary needle-catheter which is called "endobronchial intratumoral chemotherapy (EITC)" is a novel therapeutic paradigm of bronchoscopic interventional procedures. The achievement of intratumoral chemotherapy should never be considered merely an ablation technique of endobronchial tumor bulk alike the other endoscopic ablative procedures; but, it exerts most importantly an additional chemotherapeutic specific effect on malignant cells through the action of cytotoxic drug. Owing to its specific effect it has a similarity to brachytherapy and photodynamic therapy. Indeed, EITC in addition to the advantage of the initial eradication of tumoral burden inside the airway lumen, intratumoral delivery of cytotoxic drugs also provides a loco-regional neo-adjuvant therapy for irradiation or surgery. It is a real chemotherapy which differs from intravenous chemotherapy only by its route of delivery. The advantages of intratumoral injection technique include an increased precision in the local delivery of therapeutic materials, complete perfusion of the lesion, higher local tissue concentrations, as well as fewer systemic side effects.
