Şanlıurfa kırsal kesimi ilkokul öğrencilerinde tüberkülin cilt testi tarama sonuçları

Özet
MAÇ: Şanlıurfa ilinin Ceylanpınar ilçesindeki ilkokul 1. ve 2. sınıf öğrencilerinde BCG (Bacillus Calmette-Guerin) aşılama oranını, tüberkülin cilt testi (TCT) sonuçlarını, yıllık enfeksiyon riski oranını (YER) ve tüberküloz enfeksiyon prevalansını (EP) saptamayı amaçladık. GEREÇ-YÖNTEM: Toplam 3512 çocuğa BCG taraması yapıldı ve tüberkülin cilt testi uygulandı. BULGULAR: Çalışmaya alınan öğrencilerin 1746 (%49.7)?si kız, 1766 (%50.3)?ü erkek ve yaş ortalamaları 7.82±1.12 yıl idi. Öğrencilerin 2279 (%64.9)?unda BCG skarı yoktu. 1176 (%33.5)?inde tek BCG skarı, 56 (%1.6)?sında iki BCG skarı ve 1 (%0.03) öğrencide üç adet BCG skarı saptandı. İlçede eğitim gören öğrencilerin 938 (%26.7)?inde köyde eğitim gören öğrencilerin ise 1341 (%38.2)?inde BCG skarı yoktu ve köydeki öğrencilerin aşılanma oranı ilçedeki öğrencilerden anlamlı düzeyde düşüktü (p<0.001). İlçedeki öğrencilerin tüberküloz EP %5.2 ve YER %0.7, köydeki öğrencilerin EP %1.6 ve YER %0.2 olarak saptandı. SONUÇ: Çalışmamızda aşısız öğrenci oranının ülkemizdeki aynı yaş grubunda yapılmış birçok benzer çalışmalardaki aşısızlık oranlarından yüksek olduğu ve ilçenin henüz tüm yenidoğanın BCG ile aşılanması hedefinden oldukça uzak olduğu görüldü. İlçede ileriki yıllarda da bir sağlık sorunu olmaya devam edeceği görülen tbc ile mücadelede aşılama oranlarının arttırılabilmesi için sağlık hizmetlerinin bölgeye uygun olarak sunulması, tarama ve kontrol hizmetlerine önem verilmesinin gerekli olduğu kanısına varıldı.
Abstract
Aim: In this study we aimed to evaluate Bacillus Calmette Guerin (BCG) vaccination rate, results of tuberculin skin test (TST), annual infection risk (AIR) and tuberculosis infection prevalance (TIP) among the students in the first and second class of primary school in Ceylanp›nar district of fianl›urfa city. Material and methods: Evaluation for BCG scar and application of TST were performed in 3512 students. Results: Forty nine point seven percent (n=1746) of students were female and 50.3% (n=1766) were male and the mean age was 7.82±1.12 years. Concerning BCG vaccine scar, 64.9% (n=2279) of children had no scar, whereas 33.5% (n=1176) had a single scar, 1.6% (n=56) had two scars and 0.03% (n=1) had three scars. BCG scar was absent in 26.7% (n=938) of students living in district and 38.2% (n=1341) of students living in village and the vaccination rate among students living in village was significantly lower than that of students living in district (p<0.001). The tuberculosis infection prevalance (TIP) and annual infection risk for students living in distirct and village were 5.2% and 1.6%, 0.7% and 0.2%, respectively. Conclusion: In this study, we have seen that rate of students without vacination was higher than other students of the same age group involved in different studies in our country and it was far from the target for BCG vaccination among newborns in our district. As a conclusion, it is seen that tb will continue to be a health problem in the near future, so vaccination rates have to be increased to combat against tb. For this purpose, health services should be performed properly and special attention should be paid to the screening and control procedures.
