Tiroid nodüllerinde preoperatif sitolojik ve postoperatif histopatolojik sonuçlar

Özet
Çalışmada nodüler tiroid patolojisi nedeniyle preoperatif ince İğne Aspirasyon Biopsi (İİAB) ve sonrasında operasyon uygulanan hastalarda preoperatif sitoloji ile postoperatif histopatolojik sonuçların karşılaştırılması amaçlandı. Hastaların hepsine daha önceden tiroid sintigrafisi, USG ve tiroid fonksiyon testleri yapıldı. İİAB yapılan 143 olguda 131 olgu benign (%91.5), 3 olgu yetersiz (%0.02), 9 olgu malign (%0.06) rapor edildi. Histopatolojik incelemede ise 123 olguda (%86) benign guatr, 20 olguda (%13.9) malign tümör saptandı. İİAB'nin sensitivitesi %52.9, spesivitesi %100, doğruluk oranı %93.5, negatif prediktif değer %100, pozitif prediktif değer %100, yalancı negatif oran %0.05, yalancı pozitif oran saptanmadı. Sonuç olarak İİAB'nin cerrahi hasta seçiminde ve tiroid karsinomlannın ameliyat öncesi tanısında kullanılabilecek ucuz, güvenilir ve komplikasyonsuz bir tanı aracı ve tiroid ameliyatlarının preoperatif planlanmasında değerli bir yöntem olduğu kanısına varıldı.
Abstract
in this study we purposed to compare preoperative cytologic and postoperative histopathologic results of patients who had been first performed Fine Needle Aspiration Biopsy (FNAB) to thyroid nodules and then operated. AH the patients diagnosed with thyroid scintigraphy, USG and thyroid function teste. FNAB results of 143 cases were reported benign in 131 cases (%91.5), inadequate in 3 cases (%0.02) and malign in 9 cases (%0.06). in histopathologic examination these cases were found out benign guatr in 123 cases (%86), malign tümör in 20 cases (%13.9). The sensitivity of FNAB was %52.9, spesivity %100, the proportion of rightness %93.5, negative predictive value %100, positive predictive value %100, false negative ratio %0.05, and no false positive ratio. At the end we could say that FNAB is a cheap, reliable and uncomplicated way of diagnosis in selection patients and gives Information before the operation of thyroid carsinomas and valuable method preoperative planning of thyroid operations.
