Özet
Obsesif kompulsif spektrum kavramı, psikotik özellikli obsesif kompulsif bozukluk (OKB) diyebileceğimiz olgularla, obsesif kompulsif belirtiler gösteren şizofreni olguları arasında bir yere rast gelen olguların tanımlanmasının ve tedavilerinin bu tanım çerçevesinde yönlendirilmesinin yolunu açmıştır. 15 yıl farklı kurumlarda şizofreni tanısıyla izlenip tedavi edilen 46 yaşındaki bir erkek hasta, premorbid öyküsü ve sanrısal sisteminin obsesif kompulsif doğası temelinde şizo-obsesyon olgusu olarak ele alınmış ve tedavi edilmiştir. Psikofarmokolojik tedaviyle, destekleyici (supportif) bireysel psikoterapi ve kognitif-davranışçı teknikler, hastanın bireysel gereksinimi temelinde harmanlanmıştır.
Abstract
The concept of obsessive-compulsive spectrum, permits us for description and treatment ofpatients whose diagnosis are betvveen obsessive-compulsive disorder with psychotic features and schizophrenia with obsessive compulsive disorder. 46 years old patient who had been diagnosed and treated as schizophrenia in different psychiatric units, has been diagnosed and treated as schizo-obsession based on this premorbid history and delusions which has obsessive-compulsive nature. Psychopharmacologic treatment, supportive individual psychotherapy and cognitive/ behavioral therapy had been mixed based on patient's individual necessities.
