Pilonidal sinüs tedavisinde limberg flep transpozisyonu veya primer kapatma

Özet
1999-2001 yılları arasında kliniğimizde 120 hastaya pilonidal sinüs nedeni ile operasyon uygulandı. Operasyon sırasında pilonidal sinüs eksize edildikten sonra defekt küçük ise primer sütür ile kapatma işlemi uygulandı, defekt geniş ise Limberg romboid flep uygulandı. 4 hastaya enfeksiyon nedeni ile eksizyon+malsupializasyon uygulandı. Marsupializasyon uygulanan 4 hasta çalışmaya alınmadı. Vakalar; komplikasyon ve nüks yönünden takip edildi. Primer sütür tekniği uygulanan 93 hastanın 15 (% 16) inde komplikasyon, 6 (% 6.4) sında nüks gelişti. Limberg romboid flep uygulanan 23 hastanın 2 (% 8.6) sinde basit komplikasyon, birinde (% 4.3) nüks gelişti. Primer kapatma sadece küçük defetlerde kullanıldığında komplikasyon ve nüks oranının literatürden düşük olduğu, fakat yine de Limberg flep tekniğinden yüksek olduğunu bulduk. Sonuç olarak Limberg flep tekniği pilonidal sinüs tedavisinde uygun bir yöntemdir diye düşünüyoruz.
Abstract
Between 1999-2001, 120 patients were operated f ör pilonidal sinüs in our clinic. in operation; af ter excision of pilonidal sinüs, if defect was small, then primary closure was used; if defet was large, then Limberg rhomboid flap technique was used. Because of infection 4 patients were treated by excision+marsupialisation. These 4 patients are out of our trial. The cases were followed for complications and recurrens. Primary closure technique was used for 93 patients and complications were in 15 (% 16) of them, recurrens in 6 (% 6.4) of them. Limberg rhomboid flap technique was used for 23 patients and complications were in 2 (% 8.6), recurrens in (% 4.3) of them. We found that if primary closure technique was usedjust for the small defects, complication and recurrens rates were lower then the literatüre, but higher then Limberg rhomboid flap technique. As a result, we think that Limberg flap technique is usable for treatment of pilonidal sinüs.
