Renal transplantasyon sonrası üreter komplikasyonları

Özet
Amaç: Renal transplantasyonun en önemli sorunlarından biri ürolojik komplikasyonlardır. Ünitemizde yapılan böbrek nakilleri sonrası görülen ureter komplikasyon oranları, nedenleri ve tedavi modalitelerinin değerlendirilmesi amaçlanmıştır. Yöntem: Kasım 1994 -Kasım 2002 tarihleri arasında 57'si kadavra, 134'u canlı donorden, laparoskopik ve açık yöntemle alınan böbreklerle renal transplantasyon uyguladığımız toplam 191 (11-55 yaş) olgu transplantasyon sonrası gelişen üreter komplikasyonları açısından retrospektif olarak değerlendirildi. Komplikasyon gelişen ve gelişmeyen olgular yas, cinsiyet, verici tipi, nefrektomi yöntemi yönünden karşılaştırıldı. Bulgular: Kadavra vericili transplantasyonda 4 (%7.0), canlı donörlerden yapılanlarda ise 12 (%8.9) hastada olmak üzere toplam 16 (%8.3) hastada üreter komplikasyonu görüldü. Hastaların 6 (%3.8)'sında dikiş yetmezliği, bir (%0.6)'inde üreter orta bölümde transplantasyon sırasında oluşan yaralanma, 6 (%3.8) hastada ise üreteroneosistostomide obstrüksiyon ve 3 (%1.8) hastada üreter nekrozu saptandı. 16 hastanın 15'inde komplikasyon için uygulanan cerrahi girişim sonrası sorunla karşılaşılmadı. 15 hastanın komplikasyon için uygulanan ameliyattan sonra 1-73 ay arasında değişen izlem süreleri boyunca serum kreatinin düzeyleri 1.3-1.9 mg/dl arasında seyretti. Bir hasta postoperatif birinci günde kaybedildi. Sonuç: Böbreğin çıkartılması sırasında üreter damarlarının korunması üreter komplikasyonlannın önlenmesinde en önemli adımdır. Ancak komplikasyon sonrası cerrahi tedavi için erken davranılmalı ve komplikasyonların onarımlarında üretere stent konmalıdır.
Abstract
Aim: Ureteral complications are the most challenging, serious complications seen in renal transplant patients. In this study our aim was to evaluate the ureteral complications seen in our transplant patients. Methods: We reviewed the 191 patients who had had a renal transplantation between November 1994 -November 2002, for ureteral complications. 57 of the transplanted kidneys were from cadaveric and 134 were from live donors. Patients With complications were compared with patients without complications in terms of age, sex, donor and nephrectomy type. Results: Four (7.0%) cadaveric-donor and 12 (8.3%) live-donor renal transplant patients, a total of 16 (8.3%), encountered ureteral complications. 6 (3.8%) patients had suture dehiscence, one (0.6%) iatrogenic perforation, 6 (3.8%) obstruction .of ureterovesical anastomosis, and 3 (1.8%) ureter necrosis. 15 of the 16 patients whounderwent surgery for those complications left the hospital uiithout any problem. The patients were followed up for 1 to 73 months and their serum creatinine leuels were found to be between 1.3 and 1.9mg/dl within the follow-up period. One patient died on postoperative first day. Conclusion: Preservation of ureteral vascularization during nephrectomy is important in preventing urological complications. Complications should be treated immediately by surgery and ureter should be stented.
