Özet
Bu kesitsel çalışmanın iki temel amacından birincisi, sosyal güvence, anne eğitimi, gelir ve sosyal statüden oluşan sosyoekonomik durumun sağlık düzeyi ve sağlık hizmeti kullanımı üzerine etkisini saptamaktır. Araştırmanın diğer amacı ise, bireysel sağlık düzeyi belirleyicileri üzerinde etkili olan sosyoekonomik göstergeler içinde en etkili olan değişken(ler)in hangileri olduğunu ortaya koymaktır. Çalışma Ocak 2004'te Manisa'da bir sağlık ocağı bölgesinde yürütülmüştür. Bölgedeki toplam hane sayısı 2130'dur. Örnek seçiminde "büyüklüğe orantılı küme örnek seçim yöntemi" ile seçilen 203 haneye ulaşılmış, yapılandırılmış bir anketin yüz yüze görüşme şeklinde uygulanması ile araştırmanın verileri toplanmıştır. Kadınların %33'ü ilkokulu bitirmemiş ya da okuryazar değil, hane reislerinin %86.2'sinin sosyal statüsü düşük, %49.5'inin sosyal güvencesi var. “Gelirim giderimden az” diyen hane %67.5'tir. Her hangi bir sağlık sorunu yaşayanların %80.3'ü sağlık hizmeti için hekim ya da bir sağlık kurumuna başvuruyor. Son bir ayda ayakta tanı ve tedavi gerektiren bir sağlık sorunu yaşayanlarda sağlık kurumuna başvurma %49.5, kronik hastalıkta düzenli doktor kontrolünde bulunma %32.6'dır. Sağlık hizmetlerine %79.8'i zor ulaştığını söylemiştir. 0-59 ay çocukların %60.6'sı tam aşılıdır ve bu yaş grubunda çocuk ölümü %4.4'tür. Sağlık hizmeti kullanımı ve sağlık düzeyi üzerine en etkili sosyoekonomik değişken annenin eğitimidir. Bunu hane reisinin sosyal statüsü, sosyal güvence ve gelir durumu izlemiştir.
Abstract
The first one of this study's two main purposes is to determine the impact of the socioeconomic situation composed of social security, mother education, income and social situation, on use of health services and health level. The other purpose of this study is to reveal the most effective variables that affect the socioeconomic indicators on the individual characteristics of health level. The study was conducted in a health center district in January 2004 in Manisa. The total number of the households was 2130. Probability proportional to size cluster sampling approach was used for the selection of the study sample. Data were collected from residents of 203 randomly selected households. Data were collected with a structured questionnaire. 33 per cent of women hadn’t graduated from primary school or they were illiterate; 86.2 per cent of householders had lower social status; 49.5 percent of householders had social security and 67.5 per cent of householders said that its income was less than its expenditures. 80.3 per cent of the study group that experienced any sickness episode have applied to a health services facility or a doctor. And 49 per cent of those who experienced any sickness episode during the last month have applied to a health services facility. 32.6 per cent of the persons that have chronic diseases is regularly under the control of a doctor. 79.8 per cent of the study group hardly have access to the health services. 60.6 per cent of 0-59 month children are fully vaccinated and 0-59 monthly children's mortality rate is 4.4 per cent. The most influential socioeconomic variable on using of the health services and health level is mother education. Householders’social situation, social security and income, follow this.
