Özet
Bu makalede, Alzheimer Tipi Demans (ATD), görülme sıklığı ve ATD'de görülen duygusal ve bilişsel bozukluklar ve hastalığın erken dönemde yakalanmasında birinci basamaktaki hekimin öneminden söz edilmektedir. ATD'nin erken dönemde yakalanabilmesi için birinci basamakta yapılabilecek değerlendirmeler ve bu aşamada nöropsikolojik testlerin önemi vurgulanmıştır. Nöropsikolojik testlerden tanı koyma, hastanın izlenmesi, tedavi etkinliğinin değerlendirilmesi ve rehabilitasyonda yararlanılmaktadır. ATD'de meydana gelen bellek ve diğer bilişsel işlev bozukluklarını değerlendirmede kullanılabilecek, standardizasyon çalışmaları tamamlanmış nöropsikolojik testler bulunmaktadır. Bir TÜBİTAK projesi kapsamında standardizasyon çalışmaları yapılıp ülkemize kazandırılmış olan bu nöropsikolojik testler Bilişsel Potansiyeller için Nöropsikolojik Test (BİLNOT) Bataryası altında toplanmıştır (Karakaş, 2004). Makalede bu nöropsikolojik testler ve bu testlerin hangi bilişsel süreçleri değerlendirmede kullanabilecekleri de ele alınmıştır. Ayrıca bu nöropsikolojik testlerin kullanıldığı ve ATD'nin evrelerine göre farklı bellek türlerinin incelendiği bir çalışma ve bulgularından da söz edilmiştir. Birinci basamakta yapılacak doğru yönlendirme, ATD'nin erken dönemde yakalanmasını ve tedaviye daha erken başlanmasını sağlayacaktır. Böylece ATD hastasının var olan yaşam kalitesini daha uzun süre koruması olanaklı olabilir.
Abstract
In this article, the dementia of Alzheimer type (DAT), cognitive and emotional disorders seen in DAT and the importance of the general practitioners in primary health care units to detect DAT in its early stages were described. The assessments for detection of DAT in its early stages and the importance of neuropsychological tests at these stages were emphasized. Diagnosis by using neuropsychological tests, and follow-up and treatment efficacy in DAT progression were also mentioned. There are several neuropsychological tests which have been standardized completely and assessable for the memory and other cognitive disorders observed in DAT. These neuropsychological tests studied for the Turkish culture within the scope of a project of TUBITAK (Scientific and Technical Research Council of Turkey) were collected under the name of BILNOT Neuropsychological Battery (Karakaþş 2004). In this article, these neuropsychological tests and cognitive disorders which are assessable by them were mentioned. Further, a study about examining different memory types at different stages of DAT by using these neuropsychological tests and its findings were presented here. Appropriate referral of the patient in the primary health service units will provide us with early detection and chance to initiate DAT treatment in the early stages of the disease. By this way, it will be possible to protect the current life quality of DAT patients for a longer period of time.
