Özet
Bilinç; kişinin kendisini ve çevresini algılayabilmesi ve uyaranlara uygun yanıt verebilmesi durumudur. Bilinç değişikliği, acil servislere en sık başvuru nedenlerinden biridir. Bu durum, beyin hasarını ve ölümü önlemek için erken tanı ve tedavi gerektirmektedir. Bilinç düzeyini saptamak, olayın ağırlığını anlamada, bilinç düzeyindeki değişikliği izlemede ve tedaviye yanıtı değerlendirmede önemlidir. Bilinç değişikliği nedenleri yaşa göre değişmekle birlikte; sıklıkla enfeksiyonlara, zehirlenmelere ve travmalara bağlı olarak meydana gelmektedir. Bilinci kapalı bir hastanın yönetimi, etiyolojik nedenlerin tedavisi ve komplikasyonların önlenmesine dayanır. Tedavinin amacı, beyindeki hasarı en aza indirmektir. Beynin yüksek kan akımına ve sürekli oksijen sunumuna gereksinim duymasından dolayı destek tedavi öncelikli olmalıdır. Hastalığın seyri; komanın nedenine, komada kalış süresine ve komanın derinliğine bağlı olarak değişmektedir
Abstract
Consciousness is the quality or state of being aware of oneself or of an external object and the ability of reacting to stimuli. The changes in consciousness are one of the most common reasons for applying to emergency services. The situation needs early diagnosis and treatment for preventing brain injury and death. The determination of the level of consciousness is important for understanding the severity of the disease, evaluating the changes in the level of consciousness and the response to treatment. Although the causes of altered mental status vary according to age, the situation most commonly develops after infections, intoxications and trauma. Management of an unconscious patient consists of supportive treatment and prevention of complications. The aim of the treatment is to minimize damage to the brain. Because of the needs of high blood flow and continuous oxygen support for the brain, supportive therapy should be a priority. Prognosis depends on the causes leading to, the duration and the depth of the coma.
