Cerrahi Menopozun Cinsel Yaşam Üzerine Etkileri

Özet
Cerrahi menopozun cinsellik üzerindeki etkilerini tanımlamak amacıyla planlanmıştır. Yöntem: Bu çalışma İstanbul Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalı’nda, premenopozal dönemde histerektomi+bilateral ooferektomi ya da ooferektomi ameliyatı olarak, menopoza giren ve operasyondan sonra en az 6 ay geçen 20 kadının cinsel yaşamındaki değişiklikleri belirlemek amacıyla fenomenolojik yöntemin kullanıldığı nitel bir çalışma olarak planlandı. Çalışmaya dahil edilecek kadınlar arşiv taraması sonucu, amaçlı örneklem yöntemiyle seçildi. Veriler araştırmacılar tarafından literatür doğrultusunda hazırlanan tanıtıcı bilgi formu ve kadınların cerrahi menopoz sonrası cinsel yaşamındaki değişiklikleri sorgulayan açık uçlu sorulardan oluşan görüşme formu kullanılarak, yüz yüze görüşülecek dolduruldu. Elde edilen veriler kodlanarak, içerik analizi yöntemiyle konuyla ilişkili temalar oluşturuldu. Bulgular: Çalışmaya katılan kadınların 18’i histerektomi+bilateral ooferektomi, 2’si ooferektomi sonucu menopoza girdiğini ve kadınların 9’u bir süre hormon tedavisi kullandığını belirtti. Çalışmada cerrahi menopoz sonrası kadınlarda menopozal semptomların yanısıra beden imgesi değişiklikleri ve eşlerin cinsel yaşamına ilişkin olumsuz etkilerini ortaya koyan temalarda belirlendi. Sonuçlar: Cerrahi menopozala birlikte organ kaybı yaşanması, aniden ve erken yaşta menopoza girmek sorunları daha şiddetli ve karmaşık duruma getirmektedir.
Abstract
Aim: The study was planned to identify the effects of surgical menopause on sexuality. Methods: This qualitative study, in which phenomenological method was used, was conducted at Istanbul University Istanbul Medical Faculty Department of Obstetrics and Gynecology among 20 women, who have gone through menopause after undergoing hysterectomy with bilateral oophorectomy or oophorectomy in premenopausal period and spent at least 6 months after surgery, in order to identify the changes in their sexuality. The women to be included in the study were selected by searching archival records and using purposive sampling method. The data were gathered through face-to-face interviews using an information form prepared by the researchers in line with literature and an interview form composed of open-ended questions asking about the changes in women's sex life after surgical menopause. Then, the data were coded and context-related themes were formed using the content analysis method. Findings: According to their statements, the number of the women, who have gone through menopause after hysterectomy with bilateral oophorectomy and after oophorectomy was 18 and 2, respectively and 9 women have received hormone replacement therapy for a while. In the study, along with menopausal symptoms, the changes in body image and the themes indicating the negative effects on their sex life were identified among women who have gone through surgical menopause. Conclusion: Along with surgical menopause, the organ loss and a sudden entry into menopause at an early age make the problems more severe and more complicated.
