Sağlık Çalışanlarının Doğum Şekli Tercihleri ve Etkileyen Faktörler

Özet
Amaç: Araştırma, sağlık çalışanlarının doğum şekli tercihlerini ve bunu etkileyen faktörleri belirlemek amacıyla yapılmıştır. Gereç-Yöntem: Tanımlayıcı olarak planlanan araştırma, Nisan-Mayıs 2015 tarihlerinde bir üniversite hastanesinde yapılmıştır. Araştırmanın evrenini, bu hastanede çalışan, en az bir kez doğum yapmış kadın sağlık çalışanları oluşturmuştur (N=212). Araştırma 188 gönüllü katılımcı ile tamamlanmış ve evrenin %88,6’sına ulaşılmıştır (n=188). Verilerin toplanmasında anket formu kullanılmıştır. Verilerinin analizinde, tanımlayıcı istatistikler ve Ki-kare, Fisher’s Exact ve McNemar kikare testi kullanılmıştır. Bulgular: Sağlık çalışanlarının yaş ortalaması 38,55±7,38 olup %66,5’i hemşire, %22,3’i doktor ve %11,2’si ebedir. Çalışma yıl ortalaması 15,18±8,25, ilk gebelik yaşı ortalaması 26,96±3,81, yaşayan çocuk sayısı ortalaması 1,57±0,55’dir. Sağlık çalışanlarının %25,5’inin son doğumlarını vajinal doğum, %74,5’inin sezaryenle yaptıkları saptanmıştır. Vajinal doğumun tercih edilmesinin nedenleri; erken iyileşme ve eve daha çabuk dönmek/daha sağlıklı olması/daha az kanama/enfeksiyon olması (%59), daha erken emzirebilmek (%14,3), önceki doğumların vajinal olması ve daha çok çocuk isteği (%11,5) iken sezaryen doğumun tercih nedenleri; ağrı çekmemek/normal doğum korkusu (%23,7), bebeği riske atmama (%21,8), doktorun önerisi (%18,5)’dir. İlk gebelik yaşı yükseldikçe sezaryen oranlarının, çalışma yılı arttıkça vajinal doğum oranlarının arttığı bulunmuştur (p<0,005). Sonuç: Sağlık çalışanlarının sezaryen doğum tercihleri yüksek bulunmuştur. Prenatal dönemde gebelere koruyucu sağlık bakımı veren, doğumları yaptıran kadın sağlık çalışanlarının kendi doğum tercihlerinin bilinmesi ulusal sağlık hizmetlerinin planlanmasında yol gösterici olabilir
Abstract
Aim: This study aims to determine the health professionals’ preferences for mode of delivery and the factors influencing their preferences. Material and Method: This descriptive study was conducted between April and May 2015 at a university hospital. The population of the research was composed of the female health professionals working at the hospital who have given birth at least once (N=212). The research was completed with the participation of 188 volunteers and the response rate was 88,6% (N=188). The data were collected using a questionnaire. Descriptive statistics and Chi-square test were used for data analysis. Findings: The mean age of the health professionals was 38,55±7,38; 66,5% of them were nurses, 22,3% were physicians and 11,2% were midwives. The mean duration of service was 15,18±8,25 years, the mean age at first pregnancy is a 26,96±3,81and average number of living children was 1,57±0,55. It was found that of the health professionals 22,5% had a vaginal delivery and 74,5% a had Caesarean section during their recent childbirth. The reasons why the vaginal birth has been preferred were a quick recovery and coming home sooner/its being healthier/less bleeding and infection (59%), early initiation of breastfeeding (14,3%), prior vaginal delivery and desire for more children (11,5%). The reasons for preferring Caesarean section were not feeling pain/ the fear of normal delivery (23,7%), not putting the baby in jeopardy (21,8%) and physician’s recommendation (18,5%). It was found that as the age at first pregnancy increased, the rate of Caesarean delivery increased and as the duration of service increased, the rate of vaginal birth increased (p<0,005). Conclusion: The preference for Caesarean birth among health professionals was found to be high. The knowledge about own preferences for mode of delivery among female health professionals, who provide other women during the prenatal period with preventive health care and assist them at childbirth may be a guide for planning national health services.
