Özet

Bilateral koroid osteomu saptanan bir olguyu sunmak ve optik koherens tomografi (OKT) bulgularını değerlendirmek için yapılan çalışmada, yirmi yedi yaşında kadın hasta, 5 yıl önce başlayan görme azlığı nedeni ile kliniğimize başvurdu. Hastanın yapılan muayenesinde görme keskinliği sağ gözde 1 metreden parmak sayma (mps), sol gözde 2/10 düzeyinde idi. Fundus muayenesinde her iki gözde peripapiller yerleşimli ve makülayı kaplayan, sağ gözde foveal kısmı beyaz etrafı sarı-turuncu renkte, sol gözde foveal kısmı sarı-turuncu, nasal kısmı beyaz renkte, keskin sınırlı, yüzeyi düzensiz, kitle izleniyordu. Hastada koroid osteomu düşünüldü ve tanı B scan ultrasonografi (USG), bilgisayarlı tomografi (BT), fundus fluorescein anjiografi (FA) ile doğrulandı. OKT tetkiki ile değerlendirildi. Koroid osteomu selim karakterli, sıklıkla unilateral tutulumu olan ve genç kadın hastalarda görülen arka kutbun nadir bir tümörüdür. Görmeyi değişik düzeylerde etkileyebilen tümörün tanısı fundus muayenesi, USG, ve BT ile rahatlıkla konulmaktadır. Ancak son yıllarda yaygın olarak kullanılan OKT ile osteomlu bölgedeki koroidin ve retinanın anatomik yapısı daha iyi değerlendirilebilmektedir ve foveadaki bu yapının görme düzeyi ile ilişkili olduğu saptanmıştır. Sonuç olarak OKT, koroid osteomunda retina ve koroidin yapısını değerlendirmede ve tedavi planlamasında faydalı bir yöntemdir.

Abstract

We report a case of bilateral choroidal osteoma and the accompanying OCT findings A 27-year-old woman presented to our clinic with a 5-year history of bilateral visual loss. The visual acuity of the patient was counting fingers at 1 m in the right eye and 0.2 in the left. On fundus examination of both eyes a peripapillary located mass was observed. This mass in the right eye was covering the macula with a white foveal part circumscribed by a yelloworange halo; in the left eye it was covering the macula with a yellow-orange foveal part and white nasal part with an irregular surface. Choroidal osteoma was suspected. The diagnosis was confirmed by B scan ultrasonography, computer tomography, and fundus fluorescein angiography. The OCT findings were evaluated. Choroidal osteoma is a rare and benign ossifying tumor of the posterior pole of the eye that is typically seen in young women. Most commonly it occurs unilaterally. The diagnosis of this tumor, which may affect vision at different levels, can be made clinically and in combination with characteristic ultrasound and CT findings. Recently OCT has been used to evaluate the anatomical structure of the retina and choroid in choroidal osteoma. OCT is a useful method in choroidal osteoma to evaluate the anatomical structure of the retina and choroid and to identify the treatment protocol.
