Vitreoretinal cerrahi sonrası erken dönemde kornea-lens kalınlığı ve ön kamera derinliğindeki değişiklikler

Özet
Amaç: Vitreoretinal cerrahi uygulanan gözlerde ameliyat sonrası erken dönemde santral kornea kalınlığı (SKK), ön kamera derinliği (ÖKD) ve lens kalınlığı (LK) ölçümlerinin karşılaştırılması. Gereç ve Yöntem: Bu çalışmaya 19 hastanın 19 gözü dahil edildi. 10 gözde retina dekolmanı, 8 gözde vitreus hemorojisi ve 1 gözde maküla deliği bulunmaktaydı. Daha önceden herhangi bir göz cerrahisi geçiren, üveit, kornea hastalığı, glokom hikayesi olan gözler çalışmaya dahil edilmedi. Hastalar vitrektomi tekniğine göre (20 gauge (g) veya 23 g) ve kullanılan tampon maddeye göre (silikon yağı veya gaz) gruplandırıldı. Gözlerin hiçbirisinde ameliyat esnasında ön kameraya herhangi bir müdahalede bulunulmadı ve katarakt cerrahisi gerçekleştirilmedi. SKK, ÖKD ve LK ölçümleri ameliyattan 1 gün önce ve ameliyat sonrası 1.ayda tekrar edildi Parametrelerin istatistiki karşılaştırması için Wilcoxon testi kullanıldı. Bulgular: Onbir göze 20 g vitrektomi uygulandı ve 8 gözde silikon yağı 3 gözde C3F8 kullanıldı. 8 göze ise 23 g vitrektomi uygulandı ve 4 gözde silikon yağı ve 4 gözde C3F8 kullanıldı. Gözlerin hepsi fakikdi. 20 g ve 23 g vitrektomiler arasında SKK, ÖKD ve LK bakımından istatistiksel olarak anlamlı fark yoktu (p>0.05). Ayrıca, silikon yağı veya C3F8 kullanılan gözlerde SKK, ÖKD ve LK yönünden karşılaştırmasında da istatistiki olarak anlamlı bir fark bulunmadı (p>0.05). Tartışma: Vitreoretinal cerrahi ameliyat tekniğinin (20 g/23 g) veya internal tamponad maddelerin korneal kalınlık, ön kamera derinliği ve lens kalınlığı üzerinde belirgin bir etkisi cerrahi sonrası erken dönemde ortaya çıkmayabilir.
Abstract
Purpose: To compare the measurements of anterior chamber depth (ACD), lens thickness (LT), and central corneal thickness (CCT) in eyes undergoing vitreoretinal surgery in the early postoperative period. Materials and Methods: The study included 19 eyes of 19 patients. There was retinal detachment in 10 eyes, vitreous hemorrhage in 8 eyes, and a macular hole in 1 eye. The patients that had undergone previous ocular surgery, or had uveitis, or glaucoma or corneal disorders were not included. The patients were grouped according to vitrectomy technique (20 gauge(g) or 23 gauge) and internal tamponade agent (silicone oil (SO) or gas tamponade). None of the eyes underwent any surgical intervention for anterior segment or cataract surgery. ACD, LT, and CCT were measured one day before the operation and one month after the operation. The Wilcoxon test was used for the statistical analysis. Results: Eleven eyes underwent 20 g vitrectomy and SO was used in 8 eyes and C3F8 in 3 eyes. Eight eyes underwent 23 g vitrectomy and SO was used in 4 eyes and C3F8 in 4 eyes. All eyes were phakic. There was no statistically significant difference between 20 and 23 g vitrectomy with respect to ACD, LT, or CCT (p>0.05). No significant difference was found when ACD, LT, and CCT were compared between eyes with SO and those with gas tamponade (p>0.05). Conclusion: The prominent effects on ACD, LT, and CCT of the vitreoretinal surgical technique or internal tamponade agents may not appear in the early postoperative period.
