Özet

Amaç: Yaşa bağlı maküla dejeneresansında (YBMD) fotodinamik tedavi (FDT) ile FDT ile kombine intravitreal ranibizumab tedavisinin etkinliklerinin karşılaştırılması. Gereç ve Yöntem: Ocak 2005-Ocak 2010 tarihleri arasında eksüdatif tip YBMD’ye sekonder subfoveal koroidal neovaskülarizasyon (KNV) tanısı konulan 55 hastanın 56 gözü çalışmaya dahil edildi. Uygulanan tedavi şekline göre hastalar iki gruba ayrıldı. Grup I’deki 20 göze FDT ile kombine intravitreal ranibizumab uygulanırken, Grup II’deki 36 göze FDT uygulaması yapıldı. Ayrıca Grup I’deki 5 hastaya (%25) 2. doz intravitreal ranibizumab enjeksiyonu uygulandı. Hastaların tedavi öncesi en iyi düzeltilmiş görme keskinliğine bakıldı, binoküler fundus muayenesi, flöresein anjiografi ve optik kohorens tomografi (OKT) tetkikleri yapıldı. Bulgular: Ortalama takip süresi Grup I’de 21.80±11.16 ay, Grup II’de 17.47±7.42 aydı. Grup I ve Grup II’de tedavi öncesi ortalama en iyi görme keskinliği (EİDGK) sırasıyla logMAR 0.90±0.40 ve logMAR 0.98±0.43, tedavi sonrası 12. ayda sırasıyla logMAR 0.58±0.30 ve logMAR 1.18±0.59 ölçüldü. Tedavi öncesine göre 12. ayda, OKT’deki santral fovea kalınlığında (SFK) Grup I’de 113 μm, Grup II’de 120 μm azalma oldu. On iki ayda Grup I’de ortalama 1.25 seans FDT, Grup II’de ortalama 1.86 seans FDT tedavisi uygulandı. Sonuç: YBMD’ye sekonder KNV’nin tedavisinde FDT monoterapisine göre FDT ile kombine İV ranibizumab uygulaması hem mevcut görmenin korunması hem de görme artışı yönünden daha etkin bir yöntemdir. Kombinasyon tedavisi, görsel başarıyı sağlamak için uygulanan monoterapi sıklığını ve sayısını da azaltmaktadır.

Abstract

Purpose: To compare the effectiveness of photodynamic therapy (PDT) monotheraphy with PDT combined with intravitreal (IV) ranibizumab in age-related macular degeneration (AMD). Material and Methods: Fifty six eyes of 55 patients with the diagnosis of choroidal neovascularization (CNV) secondary to AMD between January 2005-January 2010 were included the study. The patients were divided into groups according to the treatment modality. Group I included 20 eyes treated with combination of PDT and IV ranibizumab and Group II included 36 eyes that were treated with PDT. In Group I five patients (25%) had second intravitreal ranibizumab injection. All patients were evaluated prior to the treatment including best corrected visual acuity testing (BCVA), binocular fundoscopy, fluorescein angiography, optical cohorence tomography (OCT). Results: The mean follow-up was 21.80±11.16 months in Group I and 21.80±11.16 months in Group II. The BCVA before the treatment in Group I and Group II was 0.90±0.40 logMAR and 0.98±0.43 logMAR respectively. The visual acuity was 0.58±0.30 logMAR and 1.18±0.59 logMAR respectively (p<0.05) at 12th month. In 12th months according to pretreatment measurements central foveal thickness was decreased 113 &#956;m in Group I and 120 &#956;m Group II. In 12th months, the mean PDT sessions number was 1.25 in Group I, 1.86 in Group II. Conclusion: Combination of IV ranibizumab with PDT is more effective than PDT monotheraphy in the treatment of CNV secondary to AMD. Combination treatment decreases the frequency and number of treatment sessions for an improved visual prognosis.
