Özet

Onüç yaşındaki kız çocuğunun yaklaşık 5 yıldan beri her iki gözünde bulanık görme şikayeti vardı. Ön segment muayenesi normal saptanan hastanın tashihli görme keskinliği sağ gözde 1/10, sol gözde 2/10 düzeyindeydi. Fundus muayenesinde bilateral tilte optik disk görünümü ve peripapiller atrofi dışında patolojik bulgu yoktu. Maküla fonksiyonları mikroperimetri ve multifokal elektroretinografi (mfERG) ile değerlendirildi. Mikroperimetri incelemesinde her iki gözde 20°’de absolu ve relative skotomlar izlenmekteydi. Hastanın mfERG’sinde santral 30 dereceyi kapsayan alanda 61 segmentten oluşan “first order kernel” dalga formlarının tüm halkalarda subnormal olduğu saptandı. Bu bulgular tilte disk sendromunda optik disk anomalisine eşlik eden maküla fonksiyon bozukluğunun bir kanıtı olarak değerlendirilebilinir. mfERG sonucunun mikroperimetrideki skotom alanları ile uyumlu olması bu görme alanı değişikliklerinden kısmen maküladaki patolojinin sorumlu olduğunu gösterebilir.

Abstract

A 13 year-old girl presented with blurred vision in both eyes for about 5 years. Best corrected visual acuity was measured as 1/10 (right eye) and 2/10 (left eye) with normal anterior segment examination. Fundus examination did not provide any pathological findings other than tilted optic disc appearance and peripapillary atrophy in both eyes. Macular functions were evaluated with microperimetry and multifocal electroretinography (mfERG). Microperimetric examination of the patient indicated absolute and relative scotomas at 20° in both eyes. On mfERG, the “first order kernel” waveforms consisting of 61 segments at central 30 degrees were found subnormal at all rings in both eyes. This findings might be the proof of macular dysfunction accompanying optic disc anomaly in tilted disc syndrome. mfERG results corresponding to scotoma areas detected by microperimetry shown that the pathology in the macula might be partially responsible for such changes in the visual field.
