Romatoid artritli olgularda serum prolaktin seviyeleri ve hastalık aktivitesi ile ilişkisi

Özet
Prolaktin ön hipofizden salgılanan bir hormondur. İmmun sistem üzerine de etkileri vardır. Bu nedenle pekçok otoimmun hastalıkta etiyopatogenezde rolü olabileceği düşünülmektedir. Bu çalışma otoimmun bir hastalık olan ve etiyopatogenezi tam olarak aydınlatılamamış olan romatoid artritli olgularda prolaktin seviyelerini tayin etmek ve hastalık aktivitesine olan etkisini araştırmak amacı ile yapıldı. Çalışmaya 1987 A.R.A kriterlerine göre romatoid artrit tanısı konulan 30 olgu ile, yaş ve cinsleri benzer 30 kontrol olgusu alındı. Romatoid artritli olguların hastalık aktivitesi değerleri kaydedildi. Sonuçta romatoid artritli 30 olgunun 9 'unda (%30) hiperprolaktinemi bulgusu vardı. Kontrol grubunda hiçbir olguda hiperprolaktinemi bulgusuna rastlanmadı. Aradaki fark istatistiksel olarak anlamlıydı (p<0.01). Romatoid artritli olguların, hastalık aktivitesi parametrelerinden ağrı, sabah tutukluğu süresi, şiş eklem sayısı, hassas eklem sayısı, eritrosit sedimentasyon hızı ile serum prolaktin seviyeleri arasında ilişki olduğu görüldü.
Abstract
Prolactin is a hormone secreted from the anterior hypophysis and has some effects on immune system. For this reason, it is considered to have a role in etiopathogenesis of several auto-immune diseases. This study was carried out to determine prolactin level and to investigate the association with the activity of the disease in rheumatoid arthritis which is an autoimmune disease and its etiopathogenesis has not been enlightened yet. Thirty cases diagnosed as rheumatoid arthritis according to 1987 A.R.A criteria and 30 control cases having similar age and sex were included into this study. Activites of the disease values of the rheumatoid arthritis cases were recorded. As a result, hyperprolactinemia was present in 9 (30%) of 30 cases with rheumatoid arthritis. No hyperprolactinemia was determined in any case of control group. The difference between prolactin values of the patient and control groups was statistically meaningful (p<0.01). A relationship was observed between serum prolactin level and the disease activity pain, duration of morning stiffness, number of swollen joints, number of tender joints, erythrocyte sedimentation rate.
