Özet

Gerek cerrahisinde gerek postoperatif rehabilitasyonunda kaydedilen ilerlemelere rağmen elin fleksör tendon yaralanmalarından sonra sonuçlar nisbeten kötü olmaya devam etmektedir. Başarısızlığın sebeplerinden biri de interfalanjiyal eklemlerde fleksiyon kontraktürlerinin gelişmesidir. Bu çalışmada, fleksör tendon onarımlarından sonra görülen ekstansiyon kayıplarını düzeltmede dinamik ekstansiyon splinti (DES)'nin etkinliği araştırıldı. Postoperatif altıncı haftada proksimal interfalanjiyal (PİF) + distal interfalanjiyal (DİF) eklemlerinde 20°'den fazla ekstansiyon kısıtlılığı olan sekiz hastadaki 20 parmak çalışmaya alındı. DEŞ'den önce her bir parmakta PİF+DİF ekstansiyon kaybı ortalama 54.0 derece iken splintten sonra bu değer ortalama 25.0 dereceye kadar düştü. Buck-Gramcko sistemine göre fonksiyonel sonuç; splintten önce 3 parmakta (% 15) iyi, 4 parmakta (% 20) orta, 13 parmakta (% 65) kötü iken, splintten sonra 3 parmakta (% 15) mükemmel, 4 parmakta (%20) iyi, 5 parmakta (% 25) orta, 8 parmakta % (40) kötü idi.

Abstract

Despite numerous advances in operative procedures and postoperative rehabilitation programmes, the results following flexor tendon repair show relatively high rates of failure. One of the causes of the failure is the development of flexion contracture at the interphalangial joints. In this study, the effectiveness of dynamic extension splint in correcting the extension deficits after flexor tendon repair was investigated. Twent digits in 8 patients who had extension deficit of more than 20 degrees in their PIP+DIP joints were included in the study. Before splinting, average extension lag at the PIP+DIP joints in each digits was 54.0°. After splinting, this value decreased to 25.0°. Functional results were evaluated by using the Buck-Gramcko system. While before splinting , 15 % of the results were good, 20 % were fair, and 65 % were poor, after dynamic extension splinting 15 % of the results were excellent, 20 % were good, 25 % were fair and 40 % were poor
