Kronik bel ve bacak ağrılı hastalarda lomber stabilizasyon egzersizleri ile kombine edilen alt lomber epidural enjeksiyonun etkisi

Özet
Bu randomize çalışma kronik bel ve bacak ağrılı hastalarda bel egzersizleri ile kombine edilen alt lomber epidural enjeksiyonun etkisini araştırmak için iki gruba ayrılmış 40 hastada yapıldı. I. grubu oluşturan 20 hastaya steroid ve anestezik madde içeren alt epidural enjeksiyon ve bel egzersiz programı uygulandı. II. gruba ise yalnızca anestezik madde ile alt epidural enjeksiyonu ve bel egzersiz programı uygulandı. Tüm hastalar tedavi öncesi, tedavi sonrası 1,6 ve 12'nci haftalarda düz bacak kaldırma testi, alt lomber Schober, vizüel analog skala ve Oswestry bel ağrısı sorgulama formu ile değerlendirildiler.Her iki grup arasında düz bacak kaldırma testi ve alt lomber Schober ölçümleri açısından fark yoktu. Bununla birlikte 6'ncı hafta vizüel analog skala, 6 ve 12'nci hafta Oswestry bel ağrısı sorgulama formu skorlarında I. gruptaki hastalar lehine anlamlı iyileşme saptandı. Sonuç olarak bel egzersizleri ile kombine edilen steroid alt epidural enjeksiyonunun kronik mekanik kökenli bel ve bacak ağrılı hastalarda etkili bir yöntem olduğunu söylenebilir.
Abstract
This study was performed randomized in order to research the efficacy of low lumbar epidural injection combined with back exercises in 40 patients that assigned to two groups. First group consisting of 20 patients were treated low lumbar epidural injection with included steroid, anesthesic substance and exercise program. Only anesthesic substance and exercise program was applied to the second group as control. All patients were evaluated before the treatment and reevaluated 1, 6 and 12<sup>th</sup> weeks as well as after the treatment by straight leg raising test, low lumbar Schober measurement, visual analog scala and Oswestry low back pain disability questionnaire. There was no significant difference in straight leg raising test and low lumbar Schober measurement between two groups. However we obtained significant improvements on visual analog scala at 6,<sup>th>/sup> week, and on Oswestry low back pain disability questionnaire scores at6<sup>th</sup> and 12<sup>th</sup> weeks. As a result, low lumbar epidural injection combined with back stabilization exercises is one of the effective therapies in chronic low back and radiating radicular pain patients.
