Spinal kord yaralanmalı hastalarda spastisite ve mesane fonksiyonu ilişkisi

Özet
Spinal kord yaralanmalarından (SKY) sonra oldukça sık görülen iki sorun spastisite (somatomotor sistem bozukluğu) ve mesane fonksiyonlarındaki bozukluklardır. Mesane otonom ve somatomotor sistemin birlikte uyumlu fonksiyon oluşturduğu özel bir organdır. Bu çalışmada amacımız farklı spinal düzeylerdeki spinal kord yaralanmalı hastalarda, spastisite ile mesane fonksiyon bozukluğu arasında bir bağlantı olup olmadığını araştırmaktı. Bu amaçla SKY'lı 42 hasta rastgele seçilerek klinik değerlendirmeye alındı. Somatomotor sistemin değerlendirilmesi için nörolojik durum ve spastisiteyi belirlemek üzere Frankel ve Ashworth skalaları kullanıldı. Mesanenin durumunu değerlendirmek için ise ürodinamik sonuçlardan yararlanıldı. İstatistik analizlerde Spearman korelasyon testi kullanıldı. Mesane fonksiyonları ile somatomotor sistem fonksiyonları arasında anlamlı korelasyon saptanmadı (p>0.05). Somatomotor sistem ve mesane fonksiyonlarının ilişkisinin saptanmamasına karşın daha homojen hasta gruplarında ve daha kapsamlı ürodinami sonuçları ile çalışıldığında daha farklı sonuçlar çıkabileceği görüşüne varıldı.
Abstract
Two frequently encountered problems in patients with spinal cord injury (SCI) are spasticity (a disorder of somatomotor system) and bladder dysfunction. Bladder is a special organ with dual innervation from both somatic and autonomic nervous system. Our purpose in this study was to search whether there was a concordance between the abnormalities in the bladder and the somatomotor system functions in patients with SCI of different neurologic levels. For this purpose 42 patients with SCI were selected randomly. For somatomotor system evaluation Frankel and Ashworth scales of each patient were recorded to determine neurological level and spasticity. The urodynamic study resuls were used for determination of bladder function.The Spearman rank correlation test was used for statistical analysis. There was no statistically significant correlation between the bladder and the somatomotor system functions. Though somatomotor system and bladder was found to act independently, it was concluded with the opinion that if the population could have been more homogenous and the urodynamic study results could be more detailed, the relationship between patient bladder function and somatomotor system might have been different.
