Özet

Otuz dort hemıplejılı hastanın deltoid, supraspinatus ve infraspinatus kaslarında bilateral olarak pozitif keskin dalga ve fibrilas.on potansiyelleri araştırıldı. Aynı zamanda hem hemiplejik, hem de normal tarafta adı geçen kaslardan motor latans ölçümleri yapıldı. 17 hastada klinik ve radyolojik subluksasyon vardı. EMG çalışmaları sonucunda hemiplejik taraftaki 3 kasından en az biride anormal bulgu saptanan hasta sayısı 17'ydi (% 50). Bilateral motor latanslarda herhangi bir patoloji saptanmadığı t;bi, subluksasyonlu ve subluksasyonsuz taraftar arasında da anlamlı bir fark ortaya konamadı. Omuz subluksasyonu ile anormal EMG bulguları arasında da istatistiksel ilişki yoktu. Sonuç olarak, hemiplejik sublukse omuzun, brakial pleksus lezyonundan sorumlu tutulmaması ve anormal EMG bulgularının bir nedeni olarak da değerlendirilmemesi gerektiği kanaatine varıldı.

Abstract

Thirty four hemiplegic patients were examined for the evidence of positive sharp waves and fibrillation potentials in the deltoid, supraspinatus and infraspinatus muscles bilaterally. Motor latency studies were also performed from the same muscles in both impaired and normal sides. Seventeen patients presented radiological and clinical subluxation. On EMG studies, abnormal potentials were noted in at least one muscle of the involved side in 17 patients (% 50). Bilateral motor latency studies did not re eal any pathology, nor did they show any significance between subluxed and non-subluxed hemiplegic shoulders. Statistical analysis indicated no association with abnormal EMG's and shoulder subluxation too. We conclude that subluxated hemiplegic shoulder is not responsible for traction injury to the brachial plexus, therefore should not be regarded as a cause of abnormal EMG findings.
