Özet

Bu çalışmada lokal soğuk uygulamanın Romatoid artrit (RA)'Iİ hastalarda inflamasyon parametrelerine etkisi araştırıldı. Bu amaçla sadece steroid olmayan antiinflamatuar ilaç (SOARİ) kullanan ve dizinde efüzyon saptanan 23 RA'li hasta çalışmaya alındı. 1 hastada soğuk ürtikeri nedeniyle tedavi kesildi. Soğuk jel paketi inflame diz eklemine günde 2 kez 15-20 dakika olmak üzere 5 gün süreyle uygulandı. Uygulamadan önce ve sonra sinovial sıvı C-reaktif protein, total protein, seruloplazrnin glukoz ve lipid peroksid değerleri ölçüldü. Ağrı şiddeti vizüel analog skala ile değerlendirildi. Tedavi sonrası total protein ve lipid peroksid değerlerinde düşme ile glukoz değerlerinde yükselme saptandı ve fark istatistiksel olarak anlamlı bulundu (p 0.05). Hastaların ağrı şiddeti tedavi son ası belirginazalma gösterdi (p<0.05). Lokal soğuk uygulamanın maliyeti düşüktür, hasta tarafından da uygulanabilir, ek olarak lokal soğuk uygulamanın analjezik etkisi sübjektif olarak saptanmıştır. RA ve diğer kronik romatizmal hastalıkların fizik tedavisinde lokal kryoterapinin önemini saptamak için daha ileri biyokimyasal çalışmalara gerek olduğu sonucuna varıldı.

Abstract

In this study, the effect of local cryotherapy on inflammation parameters in rheumatoid arthritis (RA) was evaluated. Twentythree patients with RA who had effusion in the knee joints and who were receiving nonsteroidal antiniflammatory drugs orally were included. One patient was excluded from the study who had cold urticeria following local cryotherapy. Cold pack was applied on the inflammed knee joint for fifteen-twenty minutes twice daily for five days. Synovial fluid levels of C-reactive protein, lipid peroxide, ceruloplasmin, total protein and glucose were measured before and after the therapy. Pain was evaluated with visual analog scale (VAS). Synovial fluid total protein and lipid peroxide values were significantly lower and glucose values were higher than pretreatment values. After treatment there were not statistically significant difference between the values of ceruloplasmin and CRP as compared with the pretreatment values. Pain intensity was significantly decreased after the therapy (p<0.001). The cost of local cryotherapy is low, and it can also be applied by the patient, additionally the analgesic effect of local cryotherapy was determi ed subjectively. It is concluded that further biochemical studies will be required to define the importance of local cryotherapy in :he physical therapy of RA and other chronic rheumatic diseases.
