Long-term effectiveness of biofeedback therapy with and without electrostimulation in patients with genuine stress incontinence

Özet

Bu çalışmanın amacı, stress inkontinansii hastalarda biofeedback tedavisinin elektrostimülasyonla kombine olarak veya tek başına kullanımının etkinliğini araştırmaktadır. Birinci gruptaki 13 hastaya, 28 gün süreyle günde 15-20 dk. biofeedback eşliğinde pelvik zemin egzersizleri yanısıra ev egzersiz programı uygulandı. 2. gruptaki 13 hastaya biofeedback tedavisine ilaveten 28 gün süreyle anal elektrostimülasyon uygulandı. Her iki gruptan 1 'er hasta tedaviyi tamamlayamadı. Hastalar tedavi öncesi ve sonrası; günlük miksiyon sıklığı, haftalık inkontinans sıklığı, pad test ile ölçülen idrar miktarı, sübjektif değerlendirme skalası ve biofeedback ile saptanan pelvik zemin kaslarının kontraksiyon gücüne göre değerlendirildi. Hastalar tedaviden sonra 2 yıl süreyle izlendi. Tedaviden sonra, 1. gruptaki günlük miksiyon sıklığı (p>0.05) dışındaki tüm değerlendirme parametreleri, her iki grupta istatistiksel olarak anlamlı düzelme gösterdi (p<0.01). 2 grup karşılaştırıldığında, günlük miksiyon sıklığı ve sübjektif değerlendirme skalasındaki düzelme, 2. grubun lehine istatistiksel olarak anlamlı düzeydeydi (p<0.05). Her iki grupta tedaviden sonra hastaların % 91'i iyileşti ya da düzeldi. Ancak, 2 yıllık izlemde bu.oran 1. grupta % 58, 2. grupta %66'ya düştü.

Abstract

The aim of this study was to investigate the effectiveness of biofeedback treatment with or without electrostimulation in patients with stress incontinence. 13 patients in the first group were asked to do pelvic floor exercises with biofeedback 15-20 minutes daily for 28 days, in addition to home exercise programme. 13 patients in the second group were applied anal electrostimulation for 28 days in addition to biofeedback therapy. One patient from each group was not able to complete the treatment. Patients were assessed before and after therapy for daily micturition frequency, weekly incontinence frequency, quantity of urine measured with pad test, subjective assessment scale and contraction power of the pelvic floor muscles detected by biofeedback. Patients were followed for 2 years after treatment. After the treatment, the improvement in all the assessment parameters except the daily micturiton frequency in the first group (p>0.05) were statistically significant (p<0.01) for both groups. When two groups were compared, the improvement in daily micturition frequency an subjective assessment scale were statistically significant in advantage of the second group (p<0.05). In both groups, 91 % of the patients improved after treatment. In two years follow-up period, this rate decreased to 58 % in the first and 66 % in the second group.
