Özet
Diz osteoartritinin (OA) tedavisinde hidroterapinin etkinliği araştırıldı. Yaşlan 46-69 (ört. 60±5) arasında değişen ve değişik derecelerde bilateral gonartrozu olan 51 kadın ve 4 erkek (toplam 55) hastaya rutin tetkikler ve radyolojik inceleme yapıldı. Kellegren ve Lawrence'in OA radyolojik ölçüm-metoduna göre Evre 2-4 arasında olan hastalar çalışmaya dahil edildi. Hastalar rastgele olarak tedavi ve kontrol gruplarına dağıtıldı. Tedavi grubundaki (grup 1) 30 hasta 38° C su sıcaklığında terapötik havuza 2 hafta süresince haftada 5 gün günde 20 dakika girdi. Kontrol grubundaki (grup 2) 25 hastaya herhangi bir fizik tedavi ajanı uygulanmadı. Ağrı (VAS), inaktivite katılığı (dk), 15 m yürüme süresi (dk), aktif fleksiyon derecesi (gonyometrik) ve kuadriseps kas gücü (kg) parametreleri tedavi öncesinde, tedavi sonrası 15. ve 30. günde olmak üzere her iki gruptaki hastalarda üçer kez ölçüldü ve veriler istatistiksel olarak değerlendirildi. Hidroterapi alan grupta ağrı, inaktivite katılığı, 15m yürüme süresi ve aktif fleksiyon derecesinde anlamlı bir iyileşme gözlenirken (p 0.05). Sadece kuadriseps kas gücünde her iki grupta da anlamlı düzelme gözlendi (p<0.001). Bu sonuçların ışığında OA'lı hastalarda hidroterapi uygulamasının semptomatik fayda sağlayabileceği ve seçenek bir yöntem olabileceği düşünüldü.
Abstract
The role of hydrotherapy in the treatment of knee osteoarthritis (OA) was investigated. Routine blood measurements and radiological evaluation were done in a total of 55 patients (51 women and 4 men) with an age range of 46-69 (ave. 60±5) with various degrees of bilateral gonarthrosis. Only the patients with Stage2 to 4 findings according to the radiological measurement method of Kellegren and Lawrence were included in the study. The patients were randomly distributed into treatment and control groups. In the treatment group (group 1) 30 patients received therapeutic bath at 38°C for 20 min/day, 5 days a week for two weeks. In the control group (group 2) no physical therapeutic agent was applied to 25 patients. The parameters of pain (VAS), inactivity rigidity (min), duration of 15m walking (min), active flexion range (goniometric) and quadriceps muscle strength (kg) were measured in both groups before treatment and 15 and 30 days after treatment and the results were statistically analyzed. In hydrotherapy group a significant improvement was observed in pain, inactivity rigidity, duration of 15m walking and active flexion range (p <0.001) while no significant change was found in those parameters in the control group (p>0.05). Quadriceps muscle strength improved in both groups (p <0.001). In the light of these results, it was concluded that hydrotherapy may provide symptomatic relief and may be an alternative therapy method in patients with OA.
