Romatoid artrit ve sistemik lupus eritematozuslu hastalarda serum prolaktin seviyesi ve hastalık aktivitesi ile ilişkisi

Özet

Hem romatoid artrit (RA) hem de sistemik lupus eritematozusun (SLE) etyolojisinde hormonal faktörlerin de rol aldığı iddia edilmektedir. Prolaktinin (PRL) de bu faktörler arasında yer alabileceği düşünülmektedir. Bu çalışmanın amacı, RA ve SLE'de serum PRL seviyesi ile hastalık aktivite ölçümleri arasındaki ilişkiyi araştırmaktır. Yirmibeş RA'li hasta ile bunların yaş-cinsiyet uyumlu 16 sağlıklı kontrolü ve 14 SLE'li hasta ile yaş-cinsiyet uyumlu 10 sağlıklı kontrol çalışmaya alındı. Tüm hastalara ayrıntılı öykü ve fizik muayene uygulandı. Tüm bireylerden PRL ve rutin tetkikler için sabah kan örneği alındı. RA'li hastalarda hastalık aktivitesi hassas eklem skoru, şiş eklem sayısı, vizüel analog skala ile ağrı değerlendirmesi (VASağrı), sabah tutukluğu süresi, eritrosit sedimantasyon hızı ve C-reaktif protein (CRP) seviyesi ile değerlendirildi. SLE'li hastalarda hastalık aktivitesi ise SLE hastalık aktivite indeksi (SLEHAİ) ile değerlendirildi. PRL seviyesi RA'li hastalarda daha yüksekti (p<0.05). CRP, romatoid faktör, VASağrı, hassas eklem skoru hiperprolaktinemili RA'li hastalarda daha yüksekti (p<0.05). Hiperprolaktinemik RA'li hastaların yaş ortalaması normoprolaktinemik RA'li hastalarınkinden daha düşüktü (p<0.05). SLEHAİ ile PRL seviyesi arasında anlamlı bir ilişki saptanamadı. RA'li hastalarda PRL seviyesinin yüksek olması ve hastalık aktivetesiyle paralellik göstermesi PRL 'in hem RA'in patogenezinde rol aldığını hem de hastalık şiddetini belirleyen faktörler arasında yer alabileceğini düşündürmektedir. Ancak, SLE ile PRL arasında aynı yönde bir ilişkinin saptanamamış olması her iki hastalığın farklı etyopatogenezine de işaret edebilir.

Abstract

Hormonal factors are blamed in the etiopathogenesis of both rheumatoid arthritis (RA) and systemic lupus erythematosus (SLE). Prolactin (PRL) is supposed to be amongst those factors. The aim of this study is to investigate the association of serum PRL levels with disease activity of RA and SLE. Twenty-five patients with RA and 14 patients with SLE and 16 healthy controls for RA and 10 healthy controls for SLE, matched for age and sex, were enrolled in the study. Blood samples were drawn from the all participants in the morning for the analysis of PRL and other relevant laboratory measures. Disease activity in the patients with RA was determined by tender joint score, swollen joint count, pain assessment by visual analogue scale (VASpain), duration of morning stiffness, erythrocyte sedimentation rate and C-reactive protein (CRP) levels. Disease activity in the SLE patients was evaluated by SLE disease activity index (SLEDAI). PRL levels were higher in the RA group(p<0.05). Hyperprolactinemic patients with RA showed higher CRP and rheumatoid factor levels, VASpain and tender joint scores compared to normoprolactinemic RA patients (p<0.05) and mean age of hyperprolactinemic RA patients was significantly lower than of the normoprolactinemic ones. No association was found between SLEDAI and PRL. It is suggested that PRL plays a role in the pathogenesis of RA and is associated with disease activity in those patients. As PRL has no association with SLE, it can be hypothesized that SLE and RA should have different etiopathogenetic profiles.
