Diabetik makula ödeminde intravitreal triamsinolon tedavisinin santral retina duyarlılığına etkisi

Özet
Amaç: Diabetik makula ödemi (DMÖ)'nde Intravitreal triamsinolon tedavisinin (ITT) santral retina duyarlılığı üzerindeki etkisini saptamak. Materyal ve metod: Daha önce en az 2 defa Argon Laser fotokoagulasyon (LFK) tedavisi almasına rağmen 'klinik olarak anlamlı DMO' olan 15 göz çalışmaya dahil edildi. Son LFK tedavisinden en az 6 ay sonra ITT (4 mg, I mi) uygulandı. Bilgisayarlı görme alanı (BSGA) (HFA 740) 'Santral 30-2', 'makula' eşik testleri, görme keskinliği, göziçi basıncı (GİB), biomikroskopik ve funduskopik muayeneleri ITT öncesinde ve sonrasında (L, 3. ve 6. ay) yapıldı. Bulgular: ITT sonrası sırasıyla L, 3. ve 6. aylarda görme keskinliğinin olguların % 87 (13/15 olgu),-% 73 (11/15) ve % 40 (6/15) 'ında arttığı saptandı; GİB yüksekliği (>21mmHg) % 27 (4/15), % 13 (2/15), % 7 (1/15) olguda izlendi. 'Santral 30-2' testinde iyileşme % 47 (7/15), % 33 (5/15), % 40 (6/15) olguda; 'makula eşik' testinde % 20 (3/15), % 27 (4/15), % 20 (3/15)olguda iyileşme gözlendi. Katarakt yoğunluğunda belirgin artış % 7 (1/15) olguda izlendi. İTT'ye bağlı intraoperatif komplikasyon, takiplerde retina dekolmanı, vitreus hemorajisi, endoftalmi gibi komplikasyonlara rastlanmadı. Sonuç: Argon LFK tedavisine cevap vermeyen DMÖ'inde ITT etkin ve güvenilir bir seçenek olabilir. İTT'nin etkinlik ve güvenilirliğinin tam olarak ortaya konması için daha geniş serilere ve daha uzun takibe ihtiyaç vardır.
Abstract
Purpose: To determine the effect of intravitreal triamcinolone treatment (ITT) on central retinal sensitivity in clinicly significant macular edema (CSME) in diabetic patients. Material and method: Fifteen eyes with CSME, altough previously having at least two session of argon laser photocoagulation treatment (LPT) were included in the study. ITT (4 mg) was applied at least 6 months after following the last LPT. Computerized visual field (HFA 740) (Central 30-2 and macular) threshold tests, visual acuity (VA), intra ocular pressure. (IOP) and biomicroscopic, and funduscopic examinations were performed before and after (lst,3rd- and 6th months) ITT. Results: Following the ITT (1st, 3rd and 6th months respectively); VA increased in 87 % (13/15 eyes), 73 % (11/15), and 40% (6/15) of the cases. High IOP (>21mm Hg) was observed in 27 % (4/15), 13% (2/15), 7% (1/15) of the cases. Improvement in C 30-2 test was observed in 47% (7/15), 33 % (5/15), and 40% (6/15) of the cases. Improvement in Macular threshold test was observed in 20% (3/15), 27% (4/15), and 20 % (3/15) of the cases and worsening of cataract was observed 7% (1/15) of the cases. Intraoperative complication of the ITT was not observed. Postoperatif complications such as retinal detachment, vitreous hemorrhage, endophthalmitis or other complications was not observed during the follow up period. Conclusion: ITT may be' a safe and effective alternative In DME cases that did not respond to Argon LPT but there is still need for the larger series and longer follow up periods.
