Komplike retina dekolmanlı olgularda gevşetici retinotomi

Özet
Amaç: Komplike retina dekolmanlı gözlerde gevşetici retinotomi ile birlikte vitreoretinal cerrahinin sonuçlarını değerlendirmek. Gereç ve Yöntem: Komplike retina dekolmanı nedeniyle retinotomi ile birlikte vitreoretinal cerrahi yapılan 14 olgu çalişmaya dahil edildi.5 olguya (%35.7) ilk kez vitreoretinal cerrahi uygulanırken, 9 olgu (%64.3) daha öncesinde bir veya daha fazla vitreoretinal cerrahi geçirmişti. Altı gözde (%42.8) grade D3 proliferatif vitreoretinopati, 5 gözde (%35.7) diabete bağlı fibrovasküler proliferasyon ve traksiyon,1 gözde (%7.1) eales hastalığı,1 gözde (%7.1) travma ve 1 gözde (%7.1) dejeneratif miyopi mevcuttu.On iki gözde (%85.7) intraoküler tamponad olarak silikon yağı kullanılırken, 2 gözde (%14.3) %20’lik perfluoropropane (C3F8) kullanıldı. Hastalar 6 ile 10 ay (ortalama: 8 ay) süreyle takip edildi. Bulgular: Takip süresi sonunda; retinanın anatomik durumu,11 gözde (%78.6) yatışık, 3 gözde (%21.4) dekole şeklindeydi. Postoperatif görme keskinliği 11 gözde (%78.6) preoperatif değere göre artarken, 2 gözde (%14.3) azaldı,1 gözde (%7.1) değişmeden kaldı. Postoperatif göz içi basıncı 10 (%72.6) gözde normal iken, 4 (%28.4) gözde normal sınırların altında saptandı. Postoperatif göz içi basıncı düşük olan 4 gözün 3’ünde nüks retina dekolmanı vardı. Retinanın yatışık kaldığı 11 gözün 1’inde (%9.09) hipotoni gelişti. Sonuç: Gevşetici retinotomi, komplike retina dekolmanlı gözlerde özellikle retinal kısalma da mevcut ise, intraoperatif retinanın daha kolay yatıştırılmasını sağlayan, postoperatif fonksiyonel ve anatomik başarı oranını artıran ve fitizis bulbi, neovasküler glokom gibi komplikasyonların gelişimini azaltan etkili bir tekniktir.
Abstract
Purpose: To evaluate the outcomes of relaxing retinotomy in the vitreoretinal surgery of the complicated retinal detachments. Materials and Methods: 14 patients with complicated retinal detachments who were applied retinotomy in the vitreoretinal surgery are taken into the study. While 5 cases were having their first vitreoretinal surgeries 9 cases had one or more vitreoretinal surgeries before. 6 eyes (42.8) had proliferative vitreoretinopathy, 5 eyes(%35.7) fibrovaskuler proliferation and traction due to diabetes,1 eye (%7.1) Eales disease, 1 eye (%7.1) trauma, and 1 eye (%7.1) degenerative myopia. Silicone oil was used as intraocular tamponade in 12 eyes (85.7) and %20 perfluoropropane in 2 eyes (%14.3). The patients were followed between 6 to 10 months (average:8 months). Results: At the end of the follow-up period, retina was attached in 11 eyes (%78.6). Postoperative visual acuity was better than the preoperative level in 11 eyes (%78.6), was worse in 2 eyes (%14.3) and was the same in 1 eye (%7.1). Postoperative intraocular pressure was normal in 10 (%72.6) eyes and was below normal level in 4 (%28.4) eyes. 3 of the eyes with low postoperative intraocular pressure had recurrent retinal detachments.1 (%9.09) of 11 eyes with reattached retinas postoperatively developed hypotony. Conclusion: Relaxing retinotomy, an effective technique used in the treatment of complicated retinal detachments especially in the cases with shortening of the retina, can provide an easier attachment of the retina intraoperatively, increase the functional and anatomic success and decrease the incidence of complications like phthisis bulbi and neovascular glaucoma postoperatively.
