Pars plana citrektomi cerrahisinde preemptif anestezinin etkinliği

Özet
Amaç: Pars plana vitrektomi cerrahisinde preemptif anestezinin etkinliğini araştırmak. Gereç ve Yöntem: Pars plana vitrektomi uygulanan 48 hasta prospektif olarak cerrahi sonrası ağrı ve cerrahi sırasındaki fentanil ihtiyacı açısından değerlendirildi. Bu hastalar eşit sayıda 4 gruba ayrıldı. Grup I, sadece retrobulber anestezi uygulanan hastalardan oluşturuldu. Grup II; sadece genel anestezi, grup III; genel anestezi + cerrahi öncesi retrobulber anestezi, grup IV ise, genel anestezi + cerrahi sonrası retrobulber anestezi uygulanan hastalardan oluşturuldu. Görsel analog skalası (GAS) kullanılarak her bir gruptaki hastaların cerrahi sonrası ağrı şiddetleri 1, 3, 6 ve 24. saatlerde değerlendirildi. Bulgular: GAS değerlendirmesine göre, ağrı şiddeti 1, 3, 6 ve 24. saatlerde gruplar arasında istatistiksel olarak anlamlı farklılık göstermemiştir (p>0.05). Cerrahi esnasında uygulanan fentanil miktarı III. Grupta, II. ve IV. gruplardan istatistiksel olarak daha düşük bulunmuştur (p< 0.01). Sonuç: Pars plana vitrektomi cerrahisi uygulanan hastalarda cerrahiden önce uygulanan retrobulber anestezi cerrahi sonrası ağrıda anlamlı azalmaya neden olmamakta ancak cerrahi esnasındaki fentanil ihtiyacını azaltmaktadır.
Abstract
Purpose: To describe the effect of preemtive anaesthesia in pars plana vitrectomy surgery. Materials and Methods: 48 patients who underwent pars plana vitrectomy were prospectively evaluated in terms of postoperative pain and intraoperative fentanyl requirement. These patients were divided into 4 equal groups. The patiens in group I was received retrobulber anaesthesia alone. The patients in group II was received general anaesthesia and group III was received general anaesthesia + preoperative retrobulber anaesthesia and group IV was received general anaesthesia + postoperative retrobulber anaesthesia respectively. The postoperative pain score was assessed using the Visual Analogue Scale (VAS) at 1,3,6 and 24 hours after the surgery. Results: The VAS results were not statistically different between all four groups at 1,3,6 ve 24 hours after the surgery (p>0.05). İntraoperative fentanyl requirement in group III was lower than in group II and IV (p< 0.01). Conclusion: Preoperative retrobulber anaesthesia adminisration does not reduce postoperative pain but reduces intraoperative fentanyl requirement in pars plana vitrectomy surgery.
