Fakoemülsifikasyon ile kombine limbal silikon yağı boşaltılması

Özet
Amaç: Vitreoretinal cerrahi sonrası göz içi silikon yağı tamponadı ve kataraktı olan olgularda kombine fakoemulsifikasyon ve transpupiller silikon yağı boşaltılması tekniği sonuçları ve komplikasyonlarını değerlendirmek. Gereç ve Yöntem: Fakoemülsifikasyon ve arka kapsülereksisten transpupiller yolla silikon yağı geri alınan 44 olgunun 44 gözü retrospektif olarak incelendi. 40 olguda göz içi mercek (GİM) implantasyonu yapıldı. 4 olgu GİM gücü 0 D olduğu için operasyon sonunda afak bırakıldı. Bulgular: Postoperatif nüks retina dekolman oranı %18.1 (8/44) idi. 5 olguya tekrar pars plana vitrektomi yapıldı. 2 gözde C3F8 enjeksiyonu, 3 gözde ise silikon yağı tamponatı ile retina yatıştırıldı. İris neovaskülarizasyonu ve neovasküler glokom gelişen 3 gözden 2’sine Ahmet valve implantı yapıldı. 3 olgu ise ilave müdahaleyi kabul etmedi. Son kontrolde %88.6 (39/44) retina yatışık idi. Son görme keskinliği persepsiyon pozitif ile 0.6 düzeyi arasında idi. %68.2 (30/44) gözde görmede artış sağlandı. Rekürren retina dekolmanı ve preoperatif maküler patoloji görmeyi azaltan nedenler arasında bulundu. Sonuç: Fakoemulsifikasyon ve transpupiller silikon yağı geri alınımı kombinasyonu güvenli, zaman kazandırıcı, erken görsel rehabilitasyon sağlayan bir yöntemdir. Pars plana sklerotomiye gerek duyulmaması açısından daha az travmatize edici bir cerrahidir.
Abstract
Purpose: To evaluate the technique and complications of phacoemulsification combined with silicone oil removal in cases with cataract after silicone oil endotamponade after vitreoretinal surgery. Materials and Methods: Forty-four eyes of 44 patients had phacoemulsification with removal of silicone oil through a planned posterior capsulotomy were evaluated retrospectively. An intraocular lens (IOL) was implanted in 40 eyes, 4 eyes left aphakic because the IOL power was zero. Results: The rate of retinal redetachment was 18.1% (8/44). In 5 cases, pars plana vitrectomy was done; in two of them C3F8 injection and in three of them silicone oil tamponade was necessary to reattach the retina. In two of the 3 eyes with iris neovascularization and neovascular glaucoma Ahmet valve implantation was performed. Three cases refused further treatment. Final visual acuity ranged from perception positive to 0.6. 68.2% (30/44) of the eyes had improved vision. Preexisting macular pathology and recurrent retinal detachment were the main causes for poor vision. Conclusion: Combined phacoemulsification with transpupillary silicone oil removal is a useful and time consuming procedure, with early visual rehabilitation. The procedure was less traumatic as the need for an additional sclerotomy was avoided.
