Pars plana vitrektomi sonrası korneal topografi değişimleri

Özet
Amaç: Pars plana vitrektomi (PPV) uygulanan hastalarda postoperatif dönemde kornea topografisinde meydana gelen değişikliklerin incelenmesi. Gereç ve Yöntem: İntravitreal hemoraji, maküla deliği ve retina dekolmanı tanılarıyla PPV uygulanan 15 olgunun 16 gözüne preoperatif dönem ve postoperatif 8. haftada kornea topografisi yapıldı. Tüm olgularda preoperatif ve postoperatif ölçümlerde simulasyon keratometri (Sim K) değeri ve korneanın santral kırıcılığı diyoptri cinsinden hesaplandı. Bu verilerden yararlanılarak, preoperatif ve postoperatif kornea astigmatizma değerleri belirlendi. Vektör analiz yöntemi kullanılarak cerrahiye bağlı olarak oluşturulan astigmatizma miktarı ve aksı hesaplandı. İndüklenen astigmatizmanın yönü 45-135 derece arasında olanlar kurala uygun, 0-45 ve 135-180 derece arasında olanlar kurala aykırı astigmatizma olarak kabul edildi. Bulgular: Tüm olgular değerlendirildiğinde postoperatif 8. haftada ortalama olarak 0.41 diyoptrilik kurala aykırı değişim tespit edildi. Sonuç: Vitrektomi sonrasında muhtemelen sklerotomi yerlerinin sütürasyonuna bağlı olarak, kornea topografisi üzerinde minimal bir değişim gerçekleşmektedir. Hastaların takibinde ve daha sonraki cerrahi müdehaleleri esnasında bu hususun göz önüne alınması gerektiği düşüncesindeyiz.
Abstract
Purpose: To evaluate the corneal topographic changes following pars plana vitrectomy (PPV). Materials and Methods: Computer assisted corneal topographic analysis were taken preoperatively and at postoperative 8th week to 16 eyes of 15 patients who were performed PPV with the diagnosis of intravitreal hemorrhages, macular hole and retinal detachment. Central corneal refraction and simulated keratometry values were calculated as diopters in all patients. Preoperative and postoperative astigmatic differences were measured with these values. Surgically induced astigmatism was calculated with vector analysis method. Surgically induced astigmatism between 45-135 degrees was accepted as with the rule, astigmatism between 0-45 and 135-180 degrees was accepted as against the rule astigmatism. Results: Pars plana vitrectomy induced against the rule astigmatism in all patients with a mean value of 0.41 diopter at postoperative 8th week. Conclusion: Pars plana vitrectomy induces minimal corneal topographic changes because of the suturation of the sclerostomies. This particular point must be considered in the follow up period of the patients.
