Özet
Kronik santral seröz korioretinopati (SSKR) ve SSKR’ye bağlı koroid neovaskülarizasyonu (KNV) tedavisinde foto- dinamik tedavinin (FDT) etkinliğini araştırmak Gereç ve Yöntem: S.S. Okmeydanı Eğitim ve Araştırma Has- tanesi Göz Kliniği Retina bölümüne Temmuz 2003 - Mart 2005 tarihleri arasında başvuran SSKR’li 9 olgunun 9 gözü çalışma kapsamına alındı. Olguların 2’sinde kronik SSKR ve 7’sinde SSKR ile birlikte subfoveal veya jukstafoveal KNV mevcuttu. Tanılar rutin göz muayenesi, oftalmoskopi, fun- dus muayenesi, fundus floresein anjiografi (FFA), indosi- yanin yeşili anjiografi (İSYA) ve optik koherens tomografi (OKT) yardımıyla konuldu. Olguların 8’i erkek, 1’i kadın idi. KNV’li bir olgu aynı zamanda diyabetikti. Bulgular: Olguların yaş ortalaması 45,9 (T7.7) idi. Dokuz gözün 8’inde tek FDT uygulaması ile yaklaşık 3 ay içinde sızıntının durduğu saptandı. Tek uygulamanın yeterli oldu- ğu bu 8 olgunun 7’sinde görme keskinliği ortalama 4 sıra (ETDRS eşeliyle 8-44 harf) arttı. Bunların 6’sı KNV’li olgular iken, 1’i kronik SSKR idi (44 harf artış). Kronik SSKR’li ikinci olguda görme keskinliğinde değişiklik izlenmedi. Diyabetik ve KNV’li olan bir olguya ise 2 seans FDT yapıldı. Bu olgu- da makulada skar oluşumuna bağlı 8 sıra görme azalması izlendi. Olgularda tedaviye bağlı bir komplikasyon veya yan etki görülmedi. Sonuç: Daha geniş serilere ve karşılaştırmalı çalışmalara ge- reksinim olmakla birlikte, kronik SSKR ve özellikle SSKR’ye bağlı KNV’de FDT, etkin ve güvenilir bir yöntemdir. Kronik olgularda erken tedavi, görsel prognoz açısından önemli olabilir.
Abstract
To evaluate the effect of photodynamic therapy (PDT) in the treatment of chronic central serous chorioretinopathy (CSCR) and choroidal neovascularisation (CNV) associated with CSCR. Materials and Methods: Between July 2003 and May 2005 in the Retina Department of Okmeydanı Education and Desearch Hospital Ophthalmology Clinic, 9 eyes of 9 patients were included in the study. Two of them had chronic CSCR and 7 had CSCR associated with subfoveal or juxtafovel CNV. The diagnosis was confirmed by routine ophthalmological examination, ophthalmoscopy, fundus fluorescein angiography (FFA), indocyanine green angiography (ICGA), and optical coherence tomography (OCT). Eight of them were male and 1 was female. One of the patients with CNV also had diabetes mellitus (DM). Results: Mean age of the patients was 45.9 (±7.7). In 8 of the 9 eyes the leakage stopped in about 3 months with 1 PDT session. In 8 of the 9 eyes 1 session was sufficient and in 7 of them the visual acuity improved 4 lines (8-44 letters in ETDRS). Six eyes had CNV and 1 eye had chronic CSCR (44 letters increase). In the other patient with chronic CSCR, visual acuity remained stable. Two sessions of PDT were performed in the patient with CNV and DM. An 8-line decrease in visual acuity was observed related to scar formation in the macula, in this case. No complication or side effect related to the therapy was observed. Conclusion: Although we need larger and comparative studies, PDT is an effective and safe procedure in the treatment of chronic CSCR. Early treatment may be important for visual prognosis in chronic cases.
