Patolojik miyopiye bağlı subfoveal koroid neovaskülarizasyonu olgularında intravitreal bevacizumab tedavisi

Özet
Bu çalışmanın amacı, patolojik miyopi sonucu gelişen subfoveal koroid neovaskülarizasyonlarında (KNV) intravit- real bevacizumab tedavi (İVB) uygulamasının sonuçlarını değerlendirmektir. Gereç ve Yöntem: İntravitreal bevacizumab tedavi sonuçları değerlendirilirken olguların her kontrolde görme keskinliği ve sistemik oftalmolojik muayenesi yapıldı; renkli fundus fotoğrafları, fundus floresein anjiografileri (FFA) ve optik koherens tomografileri (OKT) çekildi. Tedavi sonrası olgu- ların altıncı hafta, üçüncü ve altıncı ay kontrolleri yapıldı. Tekrar tedavi endikasyonları görme keskinliğinde azalma ile birlikte FFA’da aktif sızıntı olması ve OKT’de subretinal sıvı tespit edilmesi şeklindeydi. Sonuçlar: Patolojik miyopiye bağlı subfoveal KNV’si olan 8 olgunun 8 gözü değerlendirildi. Takip süreleri 6 ile 22 ay arasında değişmekteydi ve ortalama 11±6 aydı. İVB tedavisi sonrası son kontrol muayenelerinde hiç bir olgu- da KNV’de sızıntı görülmedi. Kontrollerde hiç bir olguda görme kaybı gözlenmedi. Son kontrol muayenesinde yedi gözde (%88) en az 1 sıra görme artışı, bunların dördünde ise (%50) en az iki sıra görme artışı saptandı. Başlangıç tedavisi (altı hafta ara ile üç doz 1.25 mg İVB) sonrası hiç bir olguda tekrar tedaviye ihtiyaç duyulmadı. İVB tedavisi sonrası herhangi bir sistemik ya da oküler yan etkiye rast- lanmadı. Tartışma: Patolojik miyopiye bağlı olarak gelişen subfoveal yerleşimli KNV olgularında İVB tedavi uygulaması ile gör- me keskinliği artmış veya korunmuş ve KNV’de gerileme kaydedilmiştir.
Abstract
To evaluate the results of intravitreal bevacizumab (IVB) therapy in patients with subfoveal choroidal neovascularization (CNV) due to pathologic myopia. Materials and Methods: Visual acuity testing, systemic ophthalmologic examination, color photographs, fluorescein angiography (FFA), and optical coherence tomography (OCT) were used to evaluate the results of IVB therapy. Follow-up examinations were scheduled at 6 weeks, and at 3 and 6 months. Indications for retreatment were active leaking CNV shown by FFA and the presence of subretinal fluid by OCT in combination visual disturbance. Results: Eight eyes of 8 patients with subfoveal CNV due to pathologic myopia were examined. Follow up ranged from 6 to 22 months with a mean of 11±6 months. At the last follow up all patients showed no leakage from CNV. No deterioration in visual acuity was observed in any case during follow-up. At the last follow-up examination, 7 eyes (88%) had gained at least 1 line of vision, 4 eyes of which had gained at least 2 lines of vision. Retreatment was not required in any patient after the initial treatment (6 weeks apart 3 times 1.25 mg IVB injection). IVB therapy did not cause any systemic or ocular complications. Conclusion: Treatment of CNV related to pathologic myopia with IVB therapy achieves cessation of fluorescein leakage from CNV and stabilizes or improves visual acuity.
