Dirençli Diyabetik Makula Ödemi Tedavisinde İntravitreal Deksametazon ve Triamsinolon Asetonid Uygulamaları Sonrası Göz İçi Basınç Artışı Görülme Sıklığının Değerlendirilmesi

Özet

Amaç: Dirençli diyabetik makula ödemi (DMÖ) olgularında düşük doz intravitreal triamsinolon asetonid (İVTA) enjeksiyonu ve yavaş alınımlı deksametazon (DEX) implantasyonunun kısa dönem göz içi basınç (GİB) değişimlerine etkisini karşılaştırmak.Gereç ve Yöntem: Bu çalışmada dirençli DMÖ nedeniyle intravitreal kortikosteroid uygulanan 57 hastanın 59 gözü retrospektif olarak incelendi. İVTA enjeksiyonu (2 mg/0.05 ml) yapılan 27 göz grup 1, DEX implantı (0.7 mg) yapılan 32 göz grup 2 olarak belirlendi. Hastaların enjeksiyon öncesi, enjeksiyon sonrası 1. ve 3. ay takiplerindeki GİB değerleri, en iyi düzeltilmiş görme keskinliği (EDGK) ve santral makula kalınlığı (SMK) ölçümleri değerlendirildi. Bulgular: Başlangıç, 1. ay ve 3. ay ortalama GİB ölçümleri grup 1'de sırasıyla 14.6±3.1, 16.14±3.2 ve 17.86±4.8 mmHg iken; grup 2'de 15.1±2.2, 17.68±4.8 ve 18.3±4.2 mmHg idi. Gruplar arasında başlangıç, 1. ay ve 3. ay GİB ölçümleri arasında anlamlı fark bulunmadı (p>0.05). Üç aylık takip sonucunda grup 1'de 7 (%25), grup 2'de 9 (%30) olguda steroide bağlı oküler hipertansiyon (OHT) gelişimi görüldü (p>0.05). Her iki grupta da başlangıca göre 1. ve 3. ay takiplerinde EDGK ve SMK ölçümlerinde istatistiksel anlamlı iyileşme saptandı (herbiri, p=0.001).Sonuç: Dirençli DMÖ tedavisinde intravitreal kortikosteroid enjeksiyonu etkili bir tedavi yöntemidir. Düşük doz İVTA enjeksiyonu ve yavaş salınımlı DEX implantı sonrası kısa dönemde benzer GİB artışı görülmektedir.

Abstract

Aim: To compare the effect of short term intraocular pressure (IOP) change after low-dose intravitreal triamcinolone acetonid (IVTA) injection and sustained-release dexamethasone (DEX) implantation in cases with persistent diabetic macular edema (DME).Materials and Methods: In this study, 59 eyes of 59 patients, who received intravitreal corticosteroids due to persistent DME were analyzed retrospectively. Group 1 was consisted of 27 eyes with IVTA injection (2 mg/0.05 ml) and group 2 had 32 eyes with DEX implantation (0.7 mg). IOP, best corrected visual acuity (BCVA) and central macular thickness (CMT) were evaluated before injection and at the 1st and 3rd months after injection. Results: The mean baseline, 1st and 3rd month IOP measurements were 14.6 ± 3.1, 16.1 ± 3.2 and 17.8 ± 4.8 mmHg in group 1, 15.1 ± 2.2, 17.6 ± 4.8 and 18.3 ± 4.2 mm Hg in group 2, respectively. Baseline, 1st and 3rd month IOP measurements were not signifi cantly different between groups (p>0.05). At the end of 3rd month follow-up, steroid induced ocular hypertension (OHT) was observed in 7 cases (%25) in group 1 and 9 cases (%30) in group 2. In each group, the improvement of BCVA and CMT from baseline were statistically signifi cant at the 1st and 3rd months follow-up (p=0.001 for each).Conclusions: Intravitreal corticosteroid injection was an effective treatment method in the treatment of persistent DME. After low-dose IVTA injection and DEX implantation, the change in IOP levels were similar in the short term follow-up.
