Epiretinal Membran Tedavisinde Pars Plana Vitrektomi İle Birlikte İç Limitan Membran Soyulmasının Retina Kalınlığına Etkisi

Özet
Amaç: İdiyopatik epiretinal membran (ERM) tedavisi için pars plana vitrektomi (PPV)+ERM+iç limitan membran (İLM) soyulması uygula- nan hastaların retina kalınlığındaki (RK) anatomik ve fonksiyonel değişiklikleri araştırmak. Gereç ve Yöntemler: Bir gözünde idiyopatik ERM tanısı alıp tedavisinde PPV+ERM+İLM soyulması gerçekleştirilen, diğer gözü sağlam olan hastalarının medikal kayıtları retrospektif olarak incelendi. RK 9 ayrı segmentte ve optik koherens tomografi (OKT) kullanılarak ölçüldü. En iyi düzeltilmiş görme keskinlikleri (EDGK) ve 9 RK alanı preoperatif ve postoperatif olarak karşılaştırıldı. Bulgular: Çalışmaya 35 hasta dahil edildi (kadın/erkek: 18/17, yaş ortalamaları: 67,40 ± 8,8 yıl). 20 hastaya eş zamanlı fakoemülsifi kasyon cerrahisi uygulanırken, 15 hasta cerrahi öncesi pseudofakik idi. 12 aylık takip sonucunda hiçbir hastada rekürrens gözlenmedi, hasta gözlerin tüm 9 RK alanları preoperatif değerlere kıyasla anlamlı miktarda incelmişti (p 0.05). Segmentler arasındaki oranlar kıyaslandığında ise postoperatif iç temporal/iç nazal RK oranının preoperatif orana ve sağlam gözün oranına kıyasla anlamlı düşük olduğu gözlendi. Eş zamanlı fakoemülsifi kasyon cerrahisi uygulanan gözler ile psödofakik gözler arasında preoperatif ve postoperatif 9 RK alanı açısından anlamlı bir fark yoktu. Her iki grupta da EDGK preoperatif değerlerle kıyasla postoperatif 12. ay anlamlı olarak artmıştı (p<0.001). Sonuç: PPV+ERM+İLM soyulmasının ERM hastaları için güvenli ve etkili olduğu görülmektedir. Eş zamanlı fakoemülsifi kasyon cerrahisinin RK üzerine etkisi psödofakik gözlerden farklı izlenmemişir.
Abstract
Purpose: To investigate anatomical and functional changes on retinal thickness (RT) in patients who underwent pars plana vitrectomy (PPV)+epiretinal membrane (ERM)+internal limiting membrane (ILM) peeling for idiopathic ERM treatment. Materials and Methods: The medical records of patients diagnosed with idiopathic ERM in one eye and treated with PPV+ERM+ILM peeling and had healthy other eye were retrospectively reviewed. RT was measured in 9 different segments using optical coherence tomography (OCT). The best corrected visual acuity (BCVA) and 9 RT areas were compared preoperatively and postoperatively. Results: Study included 35 patients (male/18 female: 17/18, mean age: 67,40 ± 8,8 years). 20 patients underwent phacoemulsifi cation simulta- neously, and 15 patients were pseudophakic before the surgery. None of the patients had recurrence after 12-month follow-up, all 9 RT areas in the patient eyes were signifi cantly decreased compared to preoperative values (p 0.05). When the ratios between the segments were compared, it was observed that the postoper- ative inner temporal/inner nasal RT ratio was signifi cantly lower than the preoperative ratio and the healthy eyes ratio. There was no signifi cant difference in preoperative and postoperative 9 RK areas between eyes with concurrent phacoemulsifi cation surgery and pseudophakic eyes. In both groups, BCVA was signifi cantly increased at 12 months postoperatively compared to preoperative values (p <0.001). Conclusion: PPV+ERM+ILM peeling seems to be safe and effective for ERM patients. The effect of concurrent phacoemulsifi cation surgery on RT is not different from that of pseudophakic eyes.
