Tip 2 Diabetes Mellitusun İç Retinal Tabakalar ve Koroidal Kalınlık Üzerindeki Etkilerinin Spektral Domain Optik Koherens Tomografi ile Değerlendirilmesi

Özet
Amaç: Tip 2 Diabetes Mellitus (DM) tanılı hastalarda iç retinal tabakalarda ve koroidal kalınlıkta meydana gelen değişikliklerin Spektral Domain Optik Koherens Tomografi (SD-OKT) tetkiki ile değerlendirilmesi. Gereç ve Yöntem: Tip 2 DM tanılı ve nonproliferatif diyabetik retinopati izlenen 43 hastanın 60 gözü (Grup 1), diyabetik retinopati izlenmeyen 47 hastanın 89 gözü (Grup 2), yaş ve cinsiyet uyumlu seçilen, tip 2 DM tanısı olmayan 46 bireyin 88 gözü (kontrol grubu) çalışmaya dahil edilmiştir. Tüm hastaların retina sinir lifi tabakası (RSLT), gangliyon hücre tabakası (GHT), iç pleksiform tabakası (İPT) kalınlıkları ve koroid kalınlığı SD-OKT ile analiz edilmiştir. Bulgular: Grup 1’de santral foveal RSLT kalınlığının diğer iki gruba göre anlamlı derecede azalmış olduğu izlenmiş olup (p=0.009, p=0.012), diğer gruplar arasında anlamlı farklılık izlenmemiştir. Grup 1’de GHT kalınlığının tüm iç kadranlarda Grup 2’ye göre anlamlı derecede azalmış olduğu izlenmiştir (p=0.005, p=0.011, p=0.002, p=0.015). Grup 1’de santral fovea, iç nazal, iç temporal, ve iç üst kadranlardaki GHT kalınlığının kontrol grubuna göre belirgin derece azalmış olduğu gösterilmiştir (p=0.015, p=0.001, p=0.010, p=0.010). İPT kalınlığının Grup 1’in iç nazal, iç temporal ve iç üst kadranlarında diğer iki gruba göre anlamlı derecede azalmış olduğu izlenmiştir (p=0.006, p=0.001, p=0.001 ve p=0.009, p=0.001, p=0.003). Gruplar arasında koroid kalınlığı açısından istatistiksel olarak anlamlı farklılık saptanmamıştır (p>0.05). Sonuç: Diyabetik hastalarda, mikrovasküler ve nörodejeneratif değişikliklere bağlı olarak ganglion hücre tabakası ve iç pleksiform tabaka kalınlıklarında belirgin incelme olduğu izlenmiştir. Koroid kalınlığı açısından ise gruplar arasında istatiksel olarak anlamlı bir farklılık olmamakla birlikte, diyabetik hastalarda koroid kalınlığının artma eğiliminde olduğu izlenmiştir.
Abstract
Purpose: To assess the changes in inner retinal layers and choroidal thickness in diabetic patients using spectral domain optical coherence tomography (SD-OCT). Materials and Methods: We examined 60 eyes of 43 diabetic patients with diabetic retinopathy (as Group 1), 89 eyes of 47 diabetic patients without diabetic retinopathy (as Group 2) and 88 eyes of 46 age and sex matched individuals without diabetes (as control group). The thickness of retinal nevre fi ber layer (RNFL), ganglion cell layer (GCL), inner plexiform layer (IPL) and choroid measured by SD-OCT. Results: In Group 1 patients, thickness of RNFL were found to be signifi cantly thinner than other groups in central subfi eld (p=0.009, p=0.012). In Group 1, thickness of GCL were found to be signifi cantly thinner than Group 2 in all inner fi elds (p=0.005, p=0.011, p=0.002, p=0.015). In Group 1, thickness of GCL were found to be signifi cantly thinner than control group in central subfi eld, inner nasal, temporal and superior fi elds (p=0.015, p=0.001, p=0.010, p=0.010). In Group 1, thickness of IPL were found to be signifi cantly thinner than other groups in inner nasal, temporal and superior fi elds (p=0.006, p=0.001, p=0.001 and p=0.009, p=0.001, p=0.003). No signifi cant difference was found between groups in terms of choroidal thickness (p>0.05). Conclusions: In diabetic patients, it was observed that the thickness of the ganglion cell and internal plexiform layer decreased as a result of microvascular and neurodegenerative changes. Although there were no statistically signifi cant differences between the groups, the choroidal thickness tend to be thicker in diabetic patients.
