The effects of inhaled corticosteroids on bone mineral density in a group of asthmatic women

Özet

İnhale kortikosteroidlerin (ks) oral steroidlere göre daha geniş terapötik kullanımı mevcuttur. Bu çalışmanın amacı inhale ks kullanan bir grup astımlı kadın hastada sağlıklı kontrol grubuyla mukayeseli olarak kemik yıkımını incelemektir. Çalışmaya klinik olarak stabil olan ve en az 3 aydır inhale ks ile tedavi edilen 40 astımlı hasta ve 40 sağlıklı kontrol vakası dahil edildi. Vakaların kemik mineral yoğunluğu (KMY) DEXA ile belirlendi. Kalsiyum, fosfor, alkalen fosfataz, parathormon, osteokalsin ve deoksipiridinolin kros bağlan gibi kemik döngüsünü gösteren değerler kaydedildi. KMY değerleri, astımlı hastalarda kontral grubuna göre tüm bölgelerde daha düşük olmasına rağmen; femur boynu, trokanter ve Ward's üçgeni gibi kalça bölgeleri dışında istatistiksel olarak anlamlı farklı bulunmadı. Yine her 2 gruba ait kemik döngüsü belirleyicileri istatistiksel olarak anlamlı farklı değildi. Hasta grubunda inhale total doz. İle hastalık süresi ve kalça KMY değerlen arasında pozitif korelasyon saplandı. Hasta grubunun kalça KMY değerleri ile deoksipiridinolin/kreatinin oranları, osteokalsin ve inhale total doz arasında ise negatif korelasyon bulundu. Sonuç olarak inhale ks kullanan astımlı kadın hastalarda kalça ve kalça alt grup KMY değerleri kontrol grubuna göre daha düşüktür. İnhale ks'lerin etkileri, kadınlardaki y aş, hormon eksikliği veya oral steroidlerin etkilerinden daha önemsiz görünse de; özellikle yüksek doz inhale ks kullanan postmenapozal kadınlarda osteoporoz riski mevcuttur.

Abstract

Inhaled corticosteroids (cs) have a wider therapeutic margin than oral cs. The aim of this study was to investigate the severity of bone loss in a group of asthmatic women who have been treated with inhaled cs, in comparison with healthy control group, Forty clinically stable asthmatic women who had been using inhaled corticosteroids regularly for at least 3 months and 40 control subjects were included to the study. Bone mineral density (BMD) was assessed by dual energy x-ray absorptiometry and biochemical markers of bone turnover including calcium, phosphorus, alkaline phosphate, parathyroid hormone, osteocalcin and deoxypyridinoline cross links were measured in patient and control groups. BMDs were lower at all sites in asthmatic patients compared to control subjects but it was not statistically significant except the hip subregions-neek of the femur, trochanter and Ward's triangle (p<0.05). We found no statistically significant difference between the levels of turnover markers of the two group. There was a positive correlation between the inhaled total dose, duration of disease and hip BMDs. A negative correlation was also found between the deoxypyridinoline / creatinine ratios, osteocalcin and inhaled total dose of steroids and BMD values of hip and lumbar spine. The clinical significance of our results show that BMD in hipsubregions of asthmatic women is lower than the healthy control group. Although the effects of in haled cs seem not to be more important than the effects of age, hormone deficiency or oral cs, especially postmenopausal women using high dose inhaled cs are at risk for osteoporosis.
