Serebrovasküler olay sonrası akut hemipleji gelişen hastalarda neglektin değerlendirilmesi

Özet
Çalışmamızda akut dönemdeki, serebrovasküler olaya (SVO) bağlı olarak hemipleji gelişen hastalarda neglekti değerlendirerek, CT'deki lezyon yeri ve klinik bulgular ile ilişkisini araştırdık. Çalışmaya SVO sonrasında hemipleji gelişen 26 hasta ve 20 sağlıklı kontrol dahil edildi. Neglekt değerlendirilmesinde Behavioural Inattention Test (BİT) formu içinde yer alan yıldız silme testi kullanıldı. Lezyon cinsi ve lokalizasyonu kraniel bilgisayarlı tomografi (BT) ile saptandı. Hemiplejik tarafın motor muayenesi Brunnstrom tekniği ile, spastisitesi Aschworth skalası ile, fonksiyonel kapasite Bartel indeksi ile, fonksiyonel ambulasyon düzeyi ise fonksiyonel ambulasyon skalası ile değerlendirildi. 26 hastanın tümünde neglekt bulundu. Nondominant sağ hemisfer lezyonlu hastalardaki neglekt evresi ve atlanan yıldız sayısı sol hemisfer lezyonlu hastalara göre anlamlı derecede yüksek idi (P<0.05). BT'deki lezyon lokalizasyonuna göre yapılan değerlendirmede sağ ve sol parieto-temporal, talamik ve bazal ganglion lezyonu olan hastaların tümünde lezyon cinsine bağlı olmaksızın neglekt saptandı. Hastalara ait klinik parametreler ile neglekt arasındaki ilişki incelendiğinde yalnızca spastisite ve neglekt evresi arasında ters yönde bir korelasyon bulunurken (r:0.75, p<0.001), atlanan yıldız sayısı ile spastisite arasında pozitif yönde anlamlı korelasyon saptandı (r:0,55, p<0.001). Sonuç olarak SVO sonucu akut hemipleji gelişen hastalarda neglekt, parieto-temporal, oksipital, talamus, pons ve bazal gangliyonlar da dahil olmak üzere kortikal ve subkortikal serebral lezyonlarda görülebilmektedir. Neglektin akut dönemde saptanmasının hekimin hasta için de uygun rehabilitasyon programım planlaması ve hastanın daha ilerideki prognozu açısından önemli olduğunu düşünmekteyiz.
Abstract
We assessed visual neglect and its association between the clinical findings and the lesion localization at the computed tomography (CT) brain scan in the patients with acute hemiplegia due to cerebrovascular accident (CVA). Twenty patients with hemiplegia following the CVA and 20 healthy control were studied. We used star cancellation test in Behavioral Inattention Test (BİT) from for the assessment of neglect. Lesion type and location were assessed using computed tomography. The motor function examination of hemiplegic patient was done according to Brunnstrom, spasticity to Aschworth, functional capacity to Bartel and functional ambulation level according functional ambulation scala. Neglect was found in all of the 26 patients. The neglect phase and the amount of the neglected stars of the patients who had right hemisphere lesion were significantly higher than the patients with the left hemisphere lesion (p <0.05). Neglect was found in all of the patients who had right and left parieto-temporal, talamic and basal ganglion lesions at CT. According to an examination which was based on neglected star numbers it was seen that patients with motor aphasia had neglected more stars (p<0.05). Negative correlation was found between spasticity and neglect phase (r=0.75, p<0.05) while there was a positive correlation between the neglected star number and the spasticity level (r=0,55, p<0.001). In conclusion, neglect can be observed in the cortical and subcortical cerebral lesions including parieto-temporal, occipital, thalamus, pons and basal ganglions in patients with acute hemiplegia due to CVA. Diagnosing neglect in acute stage is very important in order to be able to plan an appropriate rehabilitation program and for the patients' future prognosis.
