Spondilolistezisli hastalarda ilaç tedavisi ile fizik tedavi ve rehabilitasyon sonuçlarının karşılaştırılması

Spondilolistezisli hastalarda fizik tedavi ve rehabilitasyonun (FTR) önemini saptamak amacıyla, Erciyes Üniversitesi Tıp Fakültesi, Fiziksel Tıp ve Rehabilitasyon Anabilim Dalı'na başvuran ve yaş ortalaması 53.3±12.0 yıl olan 61 spondilolistezisli hasta (9 erkek, 52 kadın) çalışmaya alındı. Bunlardan 30 kişilik FTR grubuna ilaç tedavisine ek olarak FTR programı ve egzersiz tedavisi uygulanırken, 31 kişilik kontrol grubuna sadece ilaç tedavisi uygulandı. Tedavi öncesi klinik parametrelerde gruplararası farklılık yoktu (p>0.05). Her iki grupta da tedavi öncesine göre tedavi sonunda tüm klinik parametrelerde anlamlı düzelme saptandı (p 0.05), visuel analog skala (VAS), düz bacak kaldırma derecesi (DBKD) ve fonksiyonel durumda FTR grubunda kontrol grubuna göre anlamlı şekilde daha fazla iyileşme mevcuttu (VAS ve DBKD için p<0.01, fonksiyonel durum için p<0.05). Sonuçta, grade I ve II spondilolistezisli hastalarda ağrının giderilmesi, stabilitenin sağlanması ve fonksiyonel durumun iyileştirilmesinde FTR ve egzersizin faydalı olduğunu düşündük.
Abstract
To evaluate the efficacy of physical medicine and reha¬bilitation (PMR) treatment in spondy^tiftisthesis, 61 pa¬tients with spondylolisthesis (CfyfiinfWwomen, mean age 533±12,0 years), admitteff^d'Erciyes University, Medical Faculty, Department of Physical Medicine and Rehabilitation, were included in this study. PMR and exercise treatment besides medical treatment was ap¬plied in the PMR group (30 patients) and only medical treatment was applied in the control group (31 patients). In clinical parameters before treatment, there was no difference between two groups (p>0.05). At the end of the treatment, all parameters were significantly im¬proved in both groups (p<0.01). When we compared the groups after treatment; there wasn't any difference in lumbar schober improvement between the groups, how¬ever in PMR group, improvements in parameters includ¬ing visual analogue scale (VAS), straight leg raising degree (SLRD) and functional status were significantly better than the control group (p<0.01 for VAS and SLRD, p<0.05 for functional status). We concluded that PMR and exercise was helpful for pain relief, stability and functional status improvement in the patients with grade I-II spondylolisthesis.
