Özet
Double Crush Sendromu bir sinirin seyri boyunca çift kompressif lezyonun birlikteliğini tanımlayan genel bir terimdir. Patofizyolojik olarak aksoplazmik akımın etkilenmesi neden olarak gösterilmiştir. Yüksek insidansla karşılaşılan çift lezyonlardan bu mekanizmanın sorumlu olduğu düşünülmüştür. Ancak son görüşler, bu sendromun anatomik ve patofizyolojik özelliklerinin servikal radikülopati-karpal tünel sendromu veya dirsekte ulnar nöropati vb. birlikteliklerini açıklamak için kısıtlılık getirdiği şeklindedir. Bu çalışmada, boyun ve kol ağrısı şikayetleriyle başvurup servikal magnetik rezonans tetkikinde kök basısı saptanan 31 hastada radikülopati ve en sık görülen üst ekstremite periferik nöropatileri (karpal tünel sendromu, dirsekte ulnar nöropati) birlikteliği double crush hipotezinin gerektirdiği anatomik ve patofizyolojik uygunluğa göre araştırıldı. Semptomatik üst ekstremite (veya ekstremitelerde) Servikal 5-Torakal l miyotomlarında iğne EMG'si ve standart median-ulnar sinir motor ve duyu sinir iletim çalışması yapıldı. Distal lezyon yerinde elektrofizyolojik olarak saptanan patolojinin tipine göre hastalar akson kaybı, demiyelinizan, miks tip olarak 3 gruba ayrıldı. Proksimal ve distal lezyonların hasara uğrattığı kök ve periferik sinirlere göre da anatomik korelasyon incelendi. Bu değerlendirmeler sonucunda, radyolojik ve elektrofizyolojik olarak radikülopati saptanan 12 hastanın 6'sında aynı taraflı karpal tünel sendromu tespit edildi; ancak sadece l'inde anatomik ve patofizyolojik uygunluk saptandı. Sonuç olarak, double crush sendromu tanısı konulduğunda medikal ve/veya cerrahi tedaviden optimal sonuç elde edebilmek için her iki lezyonun da tedavi edilmesi gerektiğinden, sendromun standart tanı kriterlerinin olması gerektiğini düşünüyoruz
Abstract
Double crush syndrome is a general term used to define the dual compressive lesion of a nerve along its course. Pathophysiologically, the disturbance of the axoplasmic flow has been implicated as its cause. It has been postulated that this mechanism is responsible for the high incidence of dual lesions encountered. According to recent concepts, however, the anatomic and pathophysiological characteristics of this syndrome bring about some constraints in the explanation of the coexistence of cervical radiculopathies, carpal tunnel syndrome (CTS) or neuropathy of the ulnar nerve at the elbow. In this study the coexistence of radiculopathies and the most common upper extremity peripheral neuropathies (CTS, ulnar neuropathy at the elbow) was investigated according to the anatomical and pathophysiological compatibility demands of the double crush hypothesis in 31 patients, with neck and arm pain complaints and root compression detected by magnetic resonance imaging. Needle electromyography in the cervical 5-thoracic 1 myotomes and standard median-ulnar nerve motor and sensory nerve conduction studies were performed in the symptomatic upper extremity (extremities). According to the type of pathology determined electrophysiologically at the site of the distal lesion patients were divided into 3 groups: axonal loss, demyelinating, mixed type. Anatomic correlations were investigated according to involved roots and peripheral nerves. As a result of these evaluations CTS was detected in 6 of the 12 patients with radiologically and electrophysiologically diagnosed as having radiculopathies. However, anatomical and pathophysiological compatibility with the double crush syndrome was present in only 1 of the cases. It is concluded that as to obtain optimal results from the medical and/or surgical therapy in cases diagnosed as double crush syndrome, both lesions must be treated and standardized criteria are needed for the diagnosis of this syndrome.
