Kronik böbrek yetmezliği hastalarında günlük yaşam aktivitelerinin incelenmesi

Özet

Kronik böbrek yetmezliği (KBY) olan hastaların fonksiyonel durumları hakkında çok az bilgi mevcuttur. Bu hastalarda kronik hastalığa bağlı gelişen polinöropati, elektrolit inbalansı, myopati, renal osteodistrofi ve yumuşak doku kalsifikasyonu gibi komplikasyonlar hastanın günlük aktivitelerini ve yaşam kalitesini etkilemektedir. Biz de bu amaçla Başkent Üniversitesi Hastanesinde haftada üç gün dializ programı alan 60 hastaya günlük yaşam aktiviteleri (GYA) ile ilgili sorular içeren Health Assessment Questionnaire (HAQ) anketi uyguladık. Hastaların şikayetleri ile ilgili nörolojik semptom skoru (NSS), ve klinik bulgularıyla ilgili nörolojik bulgu skoru (NBS) kayıt edildi. Bu hastalardaki Hb, Htc, hastalığın başlama yaşı, hastalık süresi, polinöropati varlığı, HBV, HCV, ve rHUEPO (rekombinant Human Eritropoetin) kullanma süresi incelendi. Sonuçta hastalarımızda GYA ile NSS ve NBS arasında pozitif bir ilişki bulundu. Hb, Htc, hastalığın başlama yaşı, hastalık süresi, rHUEPO kullanma süresi, elektrofizyolojik olarak saptanan polinöropati varlığının GYA ile korelasyonu bulunamadı.

Abstract

There is little information about the functional state of patients with chronic renal failure. In these patients such complications as polyneuropathy, electrolyte imbalance, myopathy, renal osteodistrophy, and soft tissue calcifications related with the chronic disease develop and affect the daily activities and quality of life of the patients. With this purpose, we applied a Health Assessment Questionnaire (HAQ) including questions on the daily living activities to 60 patients receiving dialysis therapy at Başkent University Hospital for three days a week. Neurologic Symptom Score (NSS) related with complaints of patients and Neurologic Finding Score (NFS) related with clinical findings were recorded.In these patients Hb, Htc, age at the onset of the disease, durations of diseases, existence of polyneuropathy, HBV, HCV, and period of usage of rHUEPO (recombinant Human Erythropoietin) were studied. In conclusion, a positive relationship has been found between daily living activities and NSS as well as NFS. No correlation has been found between daily living activities and Hb, Htc, the age at the onset of the disease, duration of these disease, theperiod of usage of rHUEPO, as well as existence of polyneuropathy established electrophysiologically.
