Total kalça artroplastisi sonrası heterotopik ossifikasyonun önlenmesinde indometasin

Özet
Randomize, çift-kör, plasebo kontrollü olarak 40 hastada yapılan bu çalışmada, sementli total kalça artroplastisinden sonra heterotopik kemik formasyonunu önlemede indometasin kullanımının etkisi araştırıldı. Hastalar iki eşit gruba ayrılarak, postoperatif ilk 2 haftada günde 3 defa 25 mg. indometasin ya da plasebo verildi. Heterotopik ossifikasyonun şiddeti, operasyondan sonraki 1. yılda anteroposterior pelvis grafilerinden Brooker tarafından tanımlanan 4 dereceye göre belirlendi. Heterotopik kemik formasyonu indometasin verilen hastaların 3/20'sine karşı, plasebo verilenlerin 9/20'de gözlendi (p<0.05). Tedavi grubunda II. yada III. derecede ossifikasyon gelişmedi, oysa plasebo grubundaki 4 hastada II. ya da III. derece heterotopik ossifikasyon vardı (p<0.053). Sonuç olarak indometasin, total kalça artroplastisini takiben heterotopik ossifikasyon insidansını azaltmada etkilidir.
Abstract
The effect of indomeihacin in preventing heterötopic bone formation after cemented total hip arthroplasty was investigated in this randomized, double-blind and placebo-controlled clinical trial on 40 patients. The patients were divided into two equal group and given 25 mg 3 times indomethacin or placebo during the first 2 postoperative weeks. The severity of heterotopic bone formation, according to the four grades described by Brooker, was determined from the anteroposterior pelvis radiographs at one year after the operation. Heterotopic bone formation was observed in 9/20 pla¬cebo patients versus 3/20 indomethacin patients (p<0.05). No grade II or III ossification was developed in the treatment group, whereas 4 patients in the pla¬cebo group had grade II or III heterotopic ossification (p<0.053). In conclusion, indomethacin is effective at reducing the incidence of heterotopic ossification following total hip arthroplasty.
