İnflamatuar romatizmal hastalıklarda nitrik oksit düzeyleri ve hastalık aktivitesi ile ilişkisi

Özet
Nitrik Oksit (NO), NO sentaz izoenzim ailesi tarafından L-Arginin'den sentezlenen serbest radikal bir gazdır. Romatoid Artrit (RA), Ankilozan Spondilit (AS) gibi yüksek oranda sitokin salınımı olan kronik inflamatuar hastalıklarda serbest oksijen radikallerinin de aşırı üretimi vardır ve bu doku hasarına katkıda bulunur. NO biyolojik sıvılarda çok kısa yarı ömre sahiptir ve kolaylıkla nitrite, ardından nitrata okside olur. Bu nedenle biz hu çalışmada, RA ve AS'li hastaların se¬rum ve sinoviyal sıvılarında nitrit düzeylerini ölçtük ve sistemik aktivasyon parametreleri olan serum CRP ve ESR ile korelasyonunu araştırdık. Bu amaçla Ankara Hastanesi Fizik Tedavi ve Rehabilitasyon Kliniği'ne başvuran RA'li 23 hasta ile AS'li 11 hasta çalışmaya dahil edildi. Bu hastalar se¬rum ESR ve CRP değerlerine göre aktif ve inaktif gruplara ayrıldı. Serum nitrit düzeyi aktif RA'li ve aktif AS'li hasta grubunda inaktif hastalara göre anlamlı yüksek bulundu (p<0.01). Kontrol grubu olarak yaş ve cinsiyet uyumlu 30 OA'li hasta seçildi ve serum sinovyal sıvı nitrit düzeyleri karşılaştırıldı. İnflamatuar artrit grubundaki nitrit düzeyleri kontrol grubuna göre anlamlı yüksek bulundu (p<0.001) ve serum ESR ve CRP ile yüksek oranda korelasyon gösteriyordu (ESR r=0.79, p<0.001; CRP r=0.78, p<0.001). Sonuçlarımız göstermiştir ki, inflamatuar romatizmal hastalıklarda serum NO düzeyi artmaktadır ve bu, hastalık aktivitesinin bir göstergesi olabilir.
Abstract
Nitric oxide (NO) is a free radical gas that is synthe¬sized from L-Arginine byâf&rnity of NO synthase isoen-zyme. In chronic inflanmtumty'diseases such as Rheu¬matoid Arthritis (RA) d&fâSS&fylosing Spondylitis (AS) there is excess productmffl^XS& free radicals, that con¬tributes to tissue damage. In biological fluids, NO has a very short half-life and is easly oxidized to nitrite and subsequently nitrate. For this reason we measured nitrit levels in serum and synovia! fluid of patients with RA and AS and investigated its correlation with C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR); the parameters of systemic activity. Twenty-three pa¬tients with RA and eleven patients AS who admitted to Ankara State Hospital PM&R policlinic were encoun¬tered in this study. Disease activity was determined ac¬cording to ESR and CRP levels and according to their activity status patients were divided into 2 groups (ac¬tive and inactive groups). Serum nitrite levels was found to significantly higher in active patients of both diseases (p<0.01). Sex and age matched 30patients with osteoarthritis were selected as control group. Synovial fluid and serum nitrite lev¬els of control group were compared to the inflamma¬tory group. Nitrite levels in the inflammatory arthritis group was significantly higher than the control group (p<0.001) and this had a strong positive correlation with serum ESR and CRP (ESR r=0.79, p<0.001; CRP r=0.78,p<0.001). Our findings suggest that serum NO levels increase in inflammatory diseases and this may be an indicator of disease activity.
